Injury Waiver

I have registered my child to take dance lessons from MK Dance Center. I understand that dance class is a physical activity that may result in bodily injury. If my child does incur an injury, I will not hold MK Dance Center, its owners or affiliated teaching staff responsible for such injury, or for the payment of medical expenses accrued from said injury.
