Super Stars Waiver and Liability Form for all Super Stars Students
Please read the Acknowledgment, Waiver and Release form Liability which is printed in full below and ensure that you understand it before agreeing.
Registration forms with must be agreed by all Super Stars students or students cannot take part in classes.
Acknowledgment, Waiver and Release from Liability:
I am aware that participating in any Super Stars (SS) class/camp - theatre, drama, dance, gymnastics activities, and acrobatic dance involves inherent risk and hazards. I freely accept and fully assume all such risks and hazards.
I voluntarily agree to release Super Stars Stage School (SS), employees/coaches/directors from all liability that my child / trustee /dependent may suffer or incur as a result of participation in any class/camp held by Super Stars Cork.
As Parent or Guardian of This student that I am enrolling to classes in Super Stars Cork & Superstars Acro (SS) , if I cannot be contacted, I authorize Super Stars Stage School to seek medical services in case of serious injury or illness. I further agree to accept financial responsibility in excess of the benefits allowed by my health plan.
I understand that Super Stars Stage School employees or contractors will not assume responsibility of any lost or stolen property, or for nay bodily or personal injury consisting of or arising out of any participation in any physical training or athletic activity during my child’s class.
I confirm that I have not been advised against participation in SS events/classes and membership of SS by a health professional. I acknowledge that my statements on this membership form are being accepted by SS in consideration for allowing me to become a member of, and are being relied upon by SS and the various club and competition organizer's and administrators in permitting me to be a member of SS.
In consideration for allowing me to become a member of SS .
a) to abide by all SS and member club rules, policies and procedures as amended from time to time (“the Rulesâ€) , and I acknowledge that my membership may be revoked or suspended for violation of these rules;
1. b) to waive, release, and discharge from any and all claims, losses, or liabilities for death, personal injury, partial or permanent disability, property damage, medical expenses, theft, or damage of any kind, including economic losses, which may in the future arise out of or related to my participation in or my travelling to and from a SS sanctioned event for whatever reason, the following persons and entities: SS, SS member clubs, coaches, event sponsors, competition directors, volunteers, officers, directors, employees, representatives and agents of any of the above;
2. c) to assume sole responsibility for any and all other risks associated with participating in Sanctioned events. I acknowledge that these risks include risks that may be the result of the negligence of the persons or entities mentioned above in paragraph (b) or of other persons or entities;
3. d) not to pursue a claim of any kind against any of the persons or entities mentioned above in paragraph (b) for any of the claims, losses, or liabilities that I have waived, released, or discharged herein; and
4. e) my membership details/records being held on computer database by SS and shared with others involved in promoting/regulating the sport.
I HEREBY RELEASE, DISCHARGE AND COVENANT NOT TO SUE ALL SUPER STARS STAGE SCHOOL , ITS RESPECTIVE ADMINSTRATION, DIRECTORS, AGENTS, OFFICERS, VOLUTNEERS, AND EMPLOYEES AND LESSORS OF PREMISES ONWHICH THE ACTIVITY TAKES PLACE FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES OR DAMAGES, ON MY ACCOUNT CAUSED OR ALLEDGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE “RELEASEES†OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERTAIONS AND FUTURE AGREE THAT IF, DESPITE THIS WAIVER OF LIABILITY, AND ASSUMPTION OF RISK I, OR ANYONE ON MY BEHALF, MAKES A CLAIM AGAINST ANY OF THE RELEASEES, I WILL INDEMNMIFY, SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LOSS, LIABILITY, DAMAGE, OR COST, WHICH MAY INCUR AS A RESULT OF SUCH CLAIM.
I HAVE READ THIS WAIVER AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILTY TO THE GREATEST EXTENT ALLOWED BY LAW.
I understand that this is a legal document and have been offered the opportunity to take the document to a legal advisor, and this has been explained to me, and I hereby waive my right to do so now.
