Marietta Dance Academy Online Waiver of Liability and Risk
Please read the following waiver carefully. By checking the box and submitting this form, you acknowledge that you have read, understood, and agree to the terms and conditions outlined below. This waiver releases Marietta Dance Academy from any liability or claims that may arise during or as a result of your participation in dance classes, programs, and related activities.
By registering as a participant of the Marietta Dance Academy, the participant agrees to abide by Marietta Dance Academy’s policies and regulations for safety at all times.
Assumption of Risks:
I acknowledge that participating in dance activities involves inherent risks, including but not limited to physical exertion, muscle strain, falling, collisions, and other unforeseen circumstances. In an emergency, I consent to medical treatment the participant may require for injury or illness and authorize the designated adult instructor to obtain such treatment.
Physical and Mental Condition:
I declare that my child is physically and emotionally fit to participate in classes and programs associated with Marietta Dance Academy. My child has no medical or emotional conditions that would prevent him or her from participating in dance classes or related activities. I agree to inform the instructor or staff at Marietta Dance Academy of any changes that may affect my child’s ability to participate safely.
Release and Waiver:
In consideration of being permitted to participate in dance classes, programs, and related activities, I hereby release, discharge, and hold harmless the Marietta Dance Academy, its instructors, employees, agents, and representatives from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in these activities.
Photography and Media:
I grant Marietta Dance Academy the right to capture and use photographs, videos, or other media during dance classes, performances, or events for promotional purposes, without any compensation or approval rights.
By checking the box and submitting this form, I acknowledge that I have read and understood this Waiver of Liability and Risk, and I voluntarily agree to its terms and conditions.
