Registration for Fall Classes

In the event that the studio is forced to shut down due to COVID-19 all classes will take place online. Monthly fees that have been paid for the current month will transfer over to online classes for the remainder of that month. Should the online classes continue beyond that month, a new payment plan will be put into place. There will be no refunds.

FEES, COSTUMES, REGISTRATION
-2022/2023 Classes run from Sept -June 24. Due at time of registration: September fees, Plus a $35 Registration Fee per student Plus a $50 Costume deposit for each routine costume per student.
This fee applies to all new and returning students.
-All fees are subject to HST and are non-refundable.

-All regular monthly fees are payable on the 1st of every month. Monthly fees not paid on time are subject to a 10% late fee.
-If fees are not paid within first week of the month, the student will not be permitted to attend classes.
-Fees can be paid by Cash, Debit, cheque or E-transfer.
-We will no longer accept Visa or Master Card.
Please note, there will be a fee of $35 for all returned cheques. In the event that a cheque is returned, a cash payment must be paid immediately or your dancer will not be permitted into class.
-All accounts must be paid in full prior to year end recital in order to take part. Costumes will not be released to anyone who has a balance owing.

DRESS CODE
All dancers must follow RDC Dress Code. See Website for details.

WITHDRAWAL POLICY
-No Discounts, Refunds or transfers of any fees for missed classes for any reason such as; vacations, class trips, extracurricular events, illness or any other voluntary/involuntary absence. Any classes cancelled by the studio for reasons such as inclement weather will be re-scheduled to an alternate day and/or time. If the student withdraws before cancelled classes are made up or before month end, fees/classes are forfeited.
-Withdrawing from classes must be done a minimum of 14 days prior to monthly payment due date either by phone, in person or via email directly to Studio Owner or Artistic Director. If sufficient notice is not given, you will be responsible to make your monthly payment for the upcoming month. Failure to show up to class is not an acceptable form of withdrawal. If monthly fees have already been paid, there will be No Refunds. Costume deposits will not be refunded for any reason.
-If a student withdraws and has a balance owing, the balance must be paid and action will be taken for delinquent accounts.

RELEASE OF LIABILITY
I, The legal parent or guardian of the participant, release and hold harmless Revival Dance Company its owners, and all operators, from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Revival Dance Company, its owners and all operators or in route to or from any of said premises.

MEDICAL EMERGENCIES
I, The legal parent or guardian, gives permission to Revival Dance Company, its owners, and operators, to seek medical treatment for the participant in the event they are not able to reach me or any emergency contact provided. I hereby declare any physical/mental problems, restrictions, conditions, allergies and/or declare the participant to be in good physical and mental health.

WAIVER
I, The legal parent or guardian of the participant, agree to allow and give permission to Revival Dance Company to use any photos or video footage of my child/children on the Revival Dance Company website, Social Media sites or any promotional materials for the purpose of marketing and advertising of Revival Dance Company.
