First Aid: I agree that BAD staff may administer first aid to my child/ren if deemed necessary
Fees:
All classes must be paid for whether attended or not. No refunds will be issued
Fees are paid in a monthly, all inclusive payment via Direct Deposit. A $10 late fee will be charged for any overdue payments 7 days after the due date. This charge will be repeated every 4 days until full payment has been received.
Direct Deposits will stop once Bathurst Academy of Dance has been notified of cancellation of class attendance.
Two weeks notice must be given.
I agree to the payment terms set out above by Bathurst Academy of Dance.

Photography Permission:
I give permission for my son/daughter to be videoed or photographed whilst dancing with/for Bathurst Academy of Dance.
I give permission for these photos to be used in advertising.

Online payment fees:
1.81% + 0.30c fee will be charged to all credit cards upon payment
1% + 0.30c fee will be charged to all other online card payments

I also understand that at times teachers may use physical contact when helping improve my child's posture to assist in learning a movement. Your signature below confirms your consent of this method of teaching.
