Participation & Liability Release

PHYSICAL ACTIVITY RISK
My child, myself or other party I am responsible for is participating in dance class offered by Dreamworks Performing Arts Center (DPAC) in which they will receive information and instruction about dance and related activities. I recognize that dance and related activities requires physical exertion and movement which may be strenuous and may cause physical injury. I am fully aware of the risks and hazards involved and will do my best to prevent injuries by properly stretching, following Instructor's and associates directions, being aware and alert of my surrounds, wearing dance attire that is in good condition, well fitting and properly used. Any equipment, mats or props used must also be used properly for safety. Please ask if you are unsure. No hanging, pulling or learning on barres. Please also refrain from touching, leaning or pushing on mirrors. ONLY dance shoes (not worn outside) are allowed on dance floors. You assume all related risk while at our studio or at locations where you are attending or participating in activities. Locations and venues outside our studio may also have additional waivers and assumed safety and liabilities policies. Dancers 18 years or younger are NOT allowed to leave the studio or other related locations without prior express consent of parent AND DPAC Staff. Dancers and others who do not follow these policies may be suspended or expelled from our dance programs and related activities. Dancers will remain in the care of DPAC or affiliate until a parent/caregiver arrives. Please notify us in advance if there are special circumstances where children are not allowed to be picked up by a certain person/party. This information should be emailed for our records. We will otherwise assume the person who drops off/picks up has permission to do so. This can also include older siblings or relatives.

I will disclose any and all medical (mental and/or physical) conditions that could impact my dancer(s) performance, health, safety or those they may come into contact with. Participation includes in person at studio or out of studio venues and locations and online activities.

I further certify the additional risk associated with contagious illness. All dancers must self certify they are healthy and of sound mind to be in studio. All dancers/dance families and others participating in any way in our activities, events through DPAC or related must disclose if they tested posted for any contagious illnesses or came into contact with anyone who did. All related health & safety guidelines will be followed for our industry. We appreciate the fantastic job you all have done these last few years that kept our studio healthy and open.

I will immediately inform DPAC of any medical conditions, severe allergies, injuries or other related situations that may affect my dancer, myself or others and/or their ability to safely participate in all classes and related activities.

A doctor's note may be required to return to dance after certain illness or injury has occurred.
Injuries do require a doctor's note to participate or return in dance and related activities.
Dancers may be expected or asked to observe classes or rehearsals as feasible.

Medications (prescribed or OTC) to be used and personal protection items must be specifically approved by DPAC Management. Please email Dreamworkspac@gmail.com with any questions or concerns.
We will contact parents/caregivers if any OTC medicine, such as Tylenol is requested.
Basic First Aid Kits are available at studio for use as needed.

My child and I will respect the final decision of the faculty in regards to the safety of the child as to whether or not they may participate in classes, performances or related activities.
No refunds are given. A credit maybe provided in extenuating circumstances.

I hold harmless Dance Dreamwork, DPAC, Amanda Singer and all associated staff and workers of any liability associated with this risk whether inside the studio, outside or during online classes.

I will not hold DPAC, Miss Amanda or any associates responsible for any incidents where my dancer(s), myself or others failed to follow these guidelines either intentionally or unintentionally. I understand that DPAC and all staff will do their best to provide a safe experience but there is no expressed or implied warranty of healthy and/or safety due to the nature of illness and inherit risk during any physical activity.
Dance Dreamworks, DPAC is not responsible for dancers and others outside studio doors or those that are not current students at the studio. Dancers may not leave studio without permission from DPAC staff member.

Dancers agree to maintain proper safety at all times and younger dancers should be supervised during online classes for assistance.

Enrollment in classes automatically provides your consent to all our policies & procedures.
