Medical Release Waiver

I, the guardian of student(s) listed in parent portal waive all claims against Art in Motion Dance Studio, LLC, it's owners and employees from any type of injury or illness my child or children may incur. I release any person transporting my child to a Doctor or hospital in case of emergency. I understand as guardian I will be called immediately if an incident occurs.
