RELEASE OF LIABILITY
As a parent or legal guardian of the above-named person, I hereby give my consent to participate in all dance and aerial classes for the Young Dancers Program at Bridge For Dance. I understand that any activity involving physical movements involves certain inherent risks. I understand that is the express intent of Bridge For Dance to provide for the safety and protection of my child and, in consideration for allowing my child to use the facilities. I hereby forever waiver and release, Bridge For Dance, its officers, employees, teachers, and agents, from all liability for any damages or injuries suffered by my child while under the instruction, supervision, or control of Bridge For Dance.
I confirmed that the above-named persons are in good health and have no known physical impairments that would cause harm to the above-named persons by participation in this program. I also agree to individually provide for the possible future medical expenses, which may be incurred by my child while taking classes, training at, or performing at Bridge For Dance.
THERE IS A $20 CHARGE FOR RETURNED CHECKS. CREDITS ARE ISSUED FOR ONE YEAR FOR MEDICAL REASONS ONLY
A CERTIFICATE FROM A DOCTOR IS REQUIRED.
NO REFUNDS.
PHOTO/VIDEO PERMISSION
I give permission to use my child’s image in any promotion/advertisement for Bridge For Dance.

Bridge for Dance respects the privacy of its students and their families. We are committed to the responsible handling and protection of your personal information. We collect, use, and store your personal information in the normal course of our business, and it will not be sold or disclosed to any third parties or advertisers. Your personal information will be used solely to provide you with information about our programs, scheduling, billing, and any other services related to your participation in Bridge for Dance classes and activities. If you wish to update or remove your personal information or opt out of receiving any communications from us, please contact our office anytime.
*Bridge for Dance is not responsible for lost or stolen property or personal injury. Tuition and Fees are due as scheduled. There will be no refunds or credits for missed classes. Parents/ visitors may observe in the studio on specified weeks. While waiting in the reception area, please be aware that classes are progressing. Please pick up your child promptly after class. Notify desk staff if someone other than yourself will be picking up your child. Students should be dressed in appropriate attire and be on time for class. NO PHOTOGRAPHY OR RECORDING CLASSES ARE ALLOWED.
Bridge for Dance reserves the right to cancel classes with insufficient enrollment and to modify faculty assignments.
