Registration & Program Purchase Agreement
Please read the following and accept to proceed with Registration.
_____ I have read and understand all policies of PDT Dance Arts.
_____ I understand I am purchasing a 12 month training program for myself or my child.
_____ I understand I am fiscally obligated to pay for the full 12 month program I am purchasing; whether paid in full at registration or by making a down payment and monthly payments toward the program purchase.
_____ I will be paying for my 12 month program purchase either in full at the start of registration or with a down payment and monthly installments over 12 months
_____ I understand I am entitled to 12 months of classes included in the program I have purchased.
_____I understand there are no refunds for missed classes or ceasing to attend classes for the extent of the 12 months.
_____ I understand that make up classes are available for all missed classes. If a class is missed I am entitled to a make up class in any class of the appropriate level.
_____ I understand that if my child upgrades to a higher training program prior to the end of this program’s 12 month period, all monies paid will be applied toward purchase of the new program leaving only a remaining balance for the program expansion including additional classes.
_____ Payments toward my remaining balance will be made by the 8th of each month.
_____ If paying monthly for my program, payments not made by the 8th of each month will accrue a 15% late fee.
_____ Charges will incur until the end of my 12 month program.
_____ I understand participation in performances is optional. To participate in performances there will be a performance fee and costume fees for additional costumes.
_____ All participating students will follow the PDT Dress Code found at www.pdtdancearts.com/dresscode Failure to follow dress code will result in students observing class.
_____ All participating students will follow behavior policies. Failure to follow behavior policies will be asked to leave class without refund.
_____ I understand with any phyiscal activity there is risk of injury. I have read and completed the PDT Dance Arts Waiver of Liability form.
