Media/payment Waiver

I herby enroll myself/my student in The Dance Academy and I agree to pay to said school each month the proper amount according to terms. I understand if I am enrolling for the first time/am not returning from the previous year that a $35.00 registration fee(Fayetteville AND Lawrenceburg full year… NOT Spring or Summer session) is due upon registration and consent to pay this through email invoice/when a studio representative schedules me an appointment to come in OR online if offered upon registration. I understand tuition is a monthly payment of an annual fee regardless of classes missed, holidays, etc. divided into 12 months and that all missed classes can be made up by effort on my (parent/guardian) part. I understand that placement is according to individual accomplishments and progress. I understand no refunds will be given under any circumstance. I, the undersigned, do hereby consent and agree that The Dance Academy and it's employees have the right to take photographs, videotape, or digital recordings of my child beginning on January, 1, 2024 and ending August 1, 2025 and to use these in any and all media, now or hereafter known. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used. Photos will be used on Facebook, Instagram, Twitter, TDA Website, Local newspaper or other media outlets.
