RELEASE AND LIABILITY WAIVER
I recognize and accept ANY AND ALL RISK OF INJURY TO PERSON AND/OR PROPERTY that shall exist as a result of participating in any activity offered by TDP Dance Company. I do hereby agree to INDEMNIFY, WAIVE, RELEASE, HOLD HARMLESS, AND DISCHARGE TDP Dance Company, employees, agents, sponsors, and any individual or entity operating on behalf of TDP Dance Company from any and all liability for ANY INJURIES TO PERSON OR PROPERTY, CLAIMS, DAMAGES, OR EXPENSES that may occur or be sustained as a result of participation in any activity offered by TDP Dance Company, Inc. I understand that all sales are final and a refund will not be given. I further grant permission for TDP Dance Company, Inc. to use any photographs or videos for any legitimate purpose.
