WAIVER OF LIABILITY & PHOTO RELEASE AGREEMENTS
I hereby give my child permission to dance at Dussich Dance Studio. I waive the right to any legal action against Dussich Dance Studio for any injury sustained on studio property. I understand that I am enrolling my dancer in a program of physical activity and have agreed that my student is in good physical condition and does not suffer from any disability that would prevent or limit participation in this dance program.
I give full rights to Dussich Dance Studio and its staff to use photos and video images of me or my child to use for promotional purposes. Photos and video can be used in brochures, websites, advertisements, and other promotional material created by the studio. Photos may appear with or without names in press releases and other print advertising.
(If you do don't wish for photos or videos to be used you may ask to be excluded from this . Once Registered you can go to Waivers section of the portal and opt out or opt back in for public photos).
As a parents I will not go into a classroom unless invited by the teacher or studio owners. Student must follow the studio dress code (Available on the web or at the studio) Students Cell phones cannot be used in the classroom, If a student is injured we encourage them to watch the class when possible. Negative talk, bulling or gossip about another student, teacher or family is not tolerated. We strive for an uplifting and positive atmosphere for all. Dussich Dance studio has the right to dismiss a student or family for any infractions to this matter. I understand monthly payments are due the 1st Week of each month; No refunds will be issued for missed classes. If a class is missed the student may make up their time in a comparable class. A $15 Late Fee will be assessed after the 15th of the month if payment has not been received. No Refunds for Costumes. There will be a $20 Charge for returned checks. Charge payments will have an added processing fee. All accounts must be paid in full for the student to perform in our Annual Recital.
