Registration: A non-refundable fee of $55 per student.

Payment of Fees:
ZFL Dance Monthly Tuition will be automatically drafted from your checking account on the 1st of each month. A $15 late fee will be charged for any past due account. This includes accounts that can not be processed due to inaccurate account information or expired cards. Please check your account balance by the 5th of the month to assure your payment was successful.

COVID Policies:
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing, wearing a face mask and have, in many locations, prohibited the congregation of groups of people. Zeal For Life Dance Company has put in place preventative measures to reduce the spread of COVID-19; however, Zeal For Life Dance Company cannot guarantee that you or your child(ren) will not become infected with COVID-19.

By accepting this waiver, you agree that you will not participate in or send your child to any Zeal For Life Dance Company on-site programming, including studio classes, summer camps, workshops, etc. if you or your child show any of the following symptoms of Covid-19.

By accepting this agreement, you agree that you will not send your child to any Zeal For Life Dance Company on-site programming if they show any of the following symptoms of Covid-19. Symptoms may include, but not limited to:

● Fever of 99.5 in the last 72 hours
● Persistent cough
● Difficulty breathing
● Chills
● Muscle pain
● Sore throat
● Loss of taste or smell

By accepting this waiver, you also acknowledge that you have read the Liability Waiver and understand its contents; that you are at least eighteen (18) years old and fully competent to give your consent; That you have been sufficiently informed of the risks involved and give your voluntary consent accepting this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.

This waiver will remain effective until laws and mandates relevant to COVID-19 are lifted.
1. All participants will be screened, and temperatures taken before entering any facility. A participant who has any of the following new or worsening signs or symptoms of possible COVID-19 will be sent home. Cough – Shortness of breath or difficulty breathing – Chills – Repeated shaking with chills – Muscle pain – Headache – Sore throat – Loss of taste or smell – Diarrhea – Feeling feverish or a measured temperature greater than or equal to 100.0 degrees Fahrenheit – Known close contact with a person who is lab confirmed to have COVID-19.
2. Participants with new or worsening signs or symptoms listed above will be allowed to return until: In the case of an individual who was diagnosed with COVID-19, all three of the following criteria are met: at least 3 days (72 hours) have passed since recovery (resolution of fever without the use of fever-reducing medications); and the individual has improvement in symptoms (e.g., cough, shortness of breath); and at least 10 days have passed since symptoms first appeared; or in the case of a participant who has symptoms that could be COVID-19 and does not get evaluated by a medical professional or tested for COVID-19, the individual is assumed to have COVID-19, and the individual may not return until they have completed the same three-step criteria listed above; or if the individual has symptoms that could be COVID-19 and wants to return to camp/class before completing the above self-isolation period, the individual must obtain a medical professional’s note clearing the individual for return based on an alternative diagnosis.
3. I understand that to enter the studio premises my child must be free from COVID-19 symptoms. If, during the day, any of the following symptoms appear my child will be separated from the rest of the people in the studio. I will be contacted, and my child MUST be picked up from the studio within 15 minutes.
4. I agree that I and my child(ren) will utilize a mask while in the studio and on studio premises to reduce the risk of exposure to myself and others. Masks are required during class.
5. I further agree to utilize appropriate social distancing as well as non-contact recognition and to instruct my child(ren) in the importance of such measures.
6. I understand that during this COVID-19 Public Health Emergency I as a parent will NOT be permitted to enter the facility beyond the designated drop-off and pick-up area. I understand that this procedure change is for the safety of all persons present in the facility and to limit to the extent possible everyone’s risk of exposure.
7. I understand that students will follow CDC recommended hand washing procedures and I agree to wash or sanitize my hands after using the restroom, sneezing, and coughing, and that I will instruct my child(ren) on such preventive measures.
8. Participants will handle and care for their own equipment. Staff will not be allowed to handle.
9. All participant personal items such as bags, personal reusable water flasks/bottles and backpacks will only be allowed in the facility if placed in the marked allotted spaces.
10. I will immediately notify Zeal For Life management if I become aware of any person with whom my child or I have had contact exhibits any of the symptoms listed in Number 1 above, is advised to self-isolate, quarantine, or has tested positive, or is presumed positive for COVID-19.
11. Should there be a spike in Covid-19 cases, and we are forced into a SIP situation or we are ordered to close again, all classes will continue virtually. Tuition will remain the same for any amount of time spent doing virtual classes.

Refund Policy:
- Registration fees and tuition payments are non-refundable.
- There will be no prorating or refunds given due to missed classes.
- NO REFUNDS will be offered if we experience a closure due to COVID or other closure out of our control.

Withdrawing Policy:
You must email zfldancecompany@gmail.com thirty-days prior to your upcoming tuition payment. You will be responsible for the next payment if you do not give a thirty-day written notice. Please be aware that no prorating or refunds will be issued due to withdrawing from classes.

Unexpected Interruption of Classes:
If Zeal For Life Dance Company needs to shutdown due to COVID or other pandemic situation virtual classes will be offered in lieu of in person classes.

If Zeal For Life Dance Company must cancel some classes due to events beyond our control such as power outages, floods, etc. virtual classes will be offered. No refunds for lessons will be given.

For inclement weather, please call the studio to ensure that class will be held. A message will be left on the studio voice mail notifying you if classes will be held or not. No refunds for lessons missed due these reasons will be given.

Substitutions:
Substitutions: ZFLDC reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is ill and ZFLDC cannot arrange a substitute any missed classes will be made-up.

Dress Code/Shoes:
Combination Classes Ages 5 & Under
Practice Attire: Leotard & Tights of Any color (Do not send dancers in other attire or street clothes.). Dancers can practice in any color tap and ballet shoes. In the recital this dance season, they will need white tap & ballet shoes. The shoes can be of any dance brand.

Combination Class 5-6 Years Old
Leotard & Tights of any color. (Do not send dancers in other attire or street clothes.)
Tap Shoes: Black Lace Up Tap Shoes /Jazz Shoes: Black Jazz Shoes
Dancers can practice in any color tap and jazz shoes. In the recital this dance season they will need black tap & jazz shoes. The shoes can be of any dance brand.

Ballet (Ages 6 & Up)

Absences and Lateness:
Absences- NO REFUNDS or make up classes will be offered for missed classes.

Parent’s Responsibility to be Aware of Dates and Events: It is the responsibility of the parent to be aware of all activities, such as viewing days, recitals, extra classes, and dates studio is open or closed. ZFLDC will post all such notices on the bulletin board as well as sending reminders via email. The reminders will also be available at the studio to take home. It is the parent’s responsibility to regularly check these board to ensure they are informed. It is the responsibility of the parents to inform ZFLDC of any address, telephone, or email address change.

Care of Students: ZFLDC is not responsible for providing before or after class care for the students. Students are not to be left at the studio for excessive time before or after classes. Please be prompt when picking up your child. There will be a late pick up charge accessed to your account.

Safety:
* Safety for your child is of our utmost concern.
* All young dancers must be escorted to and from the building.
* If a health condition exist which will prevent full participation, please notify ZFLDC in writing as soon as possible.

Zeal For Life Dance Company has the right to cancel any class that has insufficient enrollment at anytime.

Injuries:
As the legal parent or guardian, I release and hold harmless Zeal For Life Dance Company, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Zeal For Life Dance Company, its owners and operators or in route to or from any of said premises.

Recital
Recital Fees: To help defray the cost of recital expenses such as the theater rental, technicians, special effects, props, sets, etc, a non-refundable $50 recital fee per dancer. The recital fee will include 2 reserved tickets per dancer to our Spring Performance, and recital T-shirt. The recital costume is $150 per dancer. Recital fees can be paid before the due date through your payment portal. If recital fees are not paid ahead of time they will be auto-drafted on the due dates listed above.

Studio Rules
* Dance shoes should not be worn outside of the building.
* There will be no food or gum permitted in the dance studio. Lobby ONLY. Please respect the studio and clean up after yourself. NO food or food containers are to be put in the bathroom trashcan. Please use the lobby trash cans only.
* Please do not leave any children unattended in the studio.
* No horseplay will be permitted in the lobby or studio.
* Dancers should arrive NO EARLIER than five minutes before scheduled class time if they are unattended by their parent guardian and MUST be picked up promptly after class.
* Please notify the studio if you will be late picking up your dancer.
* No parents are allowed to disrupt the class. Please wait for breaks between classes to use the bathroom. It breaks the dancer’s focus when parents/siblings/friends enter the class during instruction.
* Teachers reserve the right to dismiss any student from class who continuously disrupts.
* A parent/teacher conference must take place before a “dismissed student” may return to class.

Class Rules

* No jewelry may be worn to class other than small earrings.
* There will be no food or gum permitted in the studio. Water only. (No Gatorade, soda, juice, etc. )
* Any student who repeatedly disrupts the class will be dismissed from class.
