Zeal For Life Dance Company has put in place preventative measures to reduce the spread of COVID-19; however, Zeal For Life Dance Company cannot guarantee that you or your child(ren) will not become infected with COVID-19.
By accepting this waiver, you agree that you will not participate in or send your child to any Zeal For Life Dance Company on-site programming, including studio classes, summer camps, workshops, competitions etc. if you or your child show any of the following symptoms of Covid-19.
By accepting this agreement, you agree that you will not send your child to any Zeal For Life Dance Company on-site programming if they show any of the following symptoms of Covid-19. Symptoms may include, but not limited to:
● Fever of 99.5 in the last 72 hours
● Persistent cough
● Difficulty breathing
● Chills
● Muscle pain
● Sore throat
● Loss of taste or smell
By accepting this waiver, you also acknowledge that you have read the foregoing Liability Release Waiver and understand its contents; that you are at least eighteen (18) years old and fully competent to give your consent; That you have been sufficiently informed of the risks involved and give your voluntary consent accepting this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
