All Parents must read the information listed on this paper and sign below in order for their child to participate in the dance and/or gymnastics programs offered at this facility. By giving my child permission to participate in the dance and/or gymnastics program, I acknowledge that any activity involving height and motion (such as dance and gymnastics) involves the risk of injury , ranging from minor injuries (such as bruises and sprains) to serious injuries. I hereby release the Concord Dance Center, their officers, owners, employees and agents from any and all claims for damage to persons and property which might arise as a result of an accident occurring while my child is participating in the program, including class participation, competitions, and transportation to such shows and competitions. I hereby state that I have read and understand the above release, and agree to comply with the requirements and regulations of the school
In lieu of a doctor’s certificate, I hereby state that my child has had a recent physical examination and is in satisfactory condition to participate in dance and/or gymnastics and that he/she has no physical conditions except those stated on the student information form.
I give Concord Dance Center permission to use my child’s photo for promotional purposes such as brochures, website, fliers, etc. I understand that payments made to Concord Dance Center for tuition, costumes, recital tickets, etc. are non-refundable, no exceptions!
I understand that as stated in the brochure, tuition is based on an annual figure and can be paid in monthly installments, or ½ the year with a 5% discount. CDC does not offer discounts for missed classes due to illness, inclement weather, or holidays. Tuition payments are due the 1st class of the month and after the 15th, there is a $10 late fee. Bounced check fee is $25.00. Automatic payment decline fee is $25.00.
