Waiver & Release 2023

I agree to participate with All About Dance Academy LLC and programs as follows:
1. I recognize the risk of injury common to any performing arts program, and that I am participating in this program upon the express agreement and understanding that I am hereby waiving and releasing the company and its staff from and against any and all claims, actions, causes of action, damages, costs, liabilities, expense of judgements, including attorney’s fees and court costs, arising out of my participation in this program.
2. I have read the AAD studio handbook and hereby understand and will comply with all policies regarding the studio policies and Code of Conduct
3. I understand that tuition is due at the beginning of the month. ALL accounts MUST have a working card on file that will be auto
drafted on the 10th of the month. If you want to pay prior to the 10th it can be paid via cash, check
or with card online through our Dance Studio Pro Software. I understand that a student's absence does not waive tuition fees, unless there is a written medical exception.
4. I understand that if I wish to drop any class I must complete a signed Withdraw Notice prior to the next tuition to avoid responsibility for the following month’s tuition. I am aware that no prorating or refunds will be issued due to withdrawing from Classes.
5. I understand that the recital and costume fees are in addition to normal monthly tuition and are non-refundable.
6. I give All About Dance Academy, LLC permission to use my child’s photo and video imaging for advertising and marketing purposes.
7. I understand that if my account is not current, no student can participate in June 2024 recital until all tuition is paid.
8. I understand All About Dance Academy, LLC reserves the right, at its discretion, to dismiss or take disciplinary action against any student or parent/guardian/family member who breach AAD code of conduct.
9. I, the parent of an enrolled student, give the dancer’s instructor permission to physically correct the student's body position to ensure proper form and technique. Please contact the instructor with any concerns.
BY REGISTERING AND PAYING FOR ANY CLASS, EVENT, PRIVATE LESSON, OR OTHER ACTIVITY THROUGH THE STUDIO YOU ARE AGREEING TO ALL OF THE ABOVE CONDITIONS AND WAIVERS.
I have read the Waiver and Release, as well as the Studio Policies and understand them completely.
