Registration Fee - A $35.00 registration fee per dancer is due at time of enrollment. Promotions and coupons can be applied. Please make checks payable to Unbound Dance Academy.
Payment Policies - Tuition is due by the first of every month with a grace period of the 4th of that month. If you choose to add classes or withdraw from classes, you will need to notify Miss Kara two (2) weeks prior, so tuition can be adjusted.
Payments can be made via card or check made out to Unbound Dance Academy. There will be a $25 fee charged for any returned checks. At this time, due to COVID-19, we are not accepting cash.
Late Fees - A late fee of $10 will be charged to your account if payment is not received by the 4th of each month. Payments not received by the end of the month may result in dancer suspension until account is paid in full.
Photo Release - Please indicate via the online portal or with the front desk if you DO NOT give permission for your child to be photographed or videoed for studio related purposes. If you do not select the opt out of photos or videos option under the "WAIVERS" section of your online account, or notify the front desk, it is implied that you are giving permission for your child to be photographed or videoed to promote the studio. Photos may be published in our program literature, on our website, or used on local television stations or in local newspapers. Please check one of the following and sign below.
Makeup Classes - You must schedule makeups in advance through the front desk. Makeups need to be scheduled within 2 weeks of the missed class. Competition team members are required to makeup missed classes within the two week time frame to remain on team. Prorates or refunds are not given for days missed or makeup classes that cannot be completed within the 2 week time frame.
Assumption of Risk - I realize that my child is a willing participant at Unbound Dance Academy, LLC. I have communicated up to date information about any medical history including asthma, allergies, injuries or other health conditions myself or my child has that Unbound Dance Academy, LLC should know about.
Liability - I hereby waive all claims against the owner, instructors, and assistants, individually or otherwise, of Unbound Dance Academy, LLC. for any injuries he/she may sustain while attending or participating in said programs. In the event that my child sustains an injury requiring immediate medical attention, I understand that Unbound Dance Academy LLC. will make every reasonable effort to contact the individual noted above. However, in the event that the emergency contact cannot be reached, I do hereby grant Unbound Dance Academy, LLC. the authority to administer first aid and/or contact emergency personnel.
COVID-19 - I confirm that my dancer has not tested positive for COVID-19, has not had any symptoms currently related to COVID-19, has not traveled outside the United States in the last 2 weeks or had any contact with anyone who may have any symptoms of COVID-19. Symptoms include fever, cough, shortness of breath, vomiting, diarrhea, etc. Questionnaires regarding COVID-19 will be answered prior to entering the studio. If at any time you answer yes to any questions within the questionnaire, you will not be permitted into the studio and must contact the director to determine how to proceed.
