The undersigned, as a parent or legal guardian of the listed child, except the rules of Ms. Rhonda's School of Dance. I have read and understand the policies, fees and tuition associated with participation and enrollment in Ms. Rhonda’s School of Dance. As a condition of acceptance to Ms. Rhonda’s School of Dance, I agree to hold the studio and it’s employees, harmless of both and forever indefinitely, from any and all liability or legal causes of action arising from any injuries the child and/or family members and guests sustain on and off of the premises of Ms. Rhonda’s School of Dance.
