FSS Liability Waiver

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. Furthermore, I understand that in-person instruction carries some risk of transmission of infectious diseases such as COVID-19.
In recognition of this acknowledged risk of injury and infection, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Factory Street Studio, its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “FSSâ€).
I hereby agree to release FSS and hold FSS harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury and infection arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury and infection. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. The participant (if under 18) has the permission of myself, a legal guardian, to participate in Factory Street Studio events.
I further release FSS of all liabilities associated with my child’s attendance at Factory Street Studio.
FSS assumes no responsibility for children on the premises before, after, or between their specified class times. Further, I have read the current Studio Policies and understand the expectations for my student’s participation in class.
Please EMAIL any medical or special needs that we should know about, medications the participant is taking, and any allergies or special medical instructions. We also encourage (but do not require) you to disclose any Learning, Developmental, Mood or Sensory disorders your child has so that teachers can meet your student's learning needs. Please email this information to director@factorystreetstudio.org with the subject line: Medical/Learning needs. This information will remain confidential. It will only be shared on a need-to-know basis with appropriate teachers.
We firmly believe that our classes are made stronger through inclusivity. Our goals in encouraging your to share this information are (1) to nurture an empathetic and supportive environment
for all students and (2) to insure that our teachers are prepared for your child’s behaviors and triggers so that they can plan and respond in a supportive manner.
