I have read, understand and agree to comply to the policies as stated in Stars on 9 Dance Center 2023-2024 Handbook and Guidelines listed in the Student File Folder in this Family Portal. I understand that if I choose to drop a class, this written notification must be made to Stars on 9 Dance Center office in writing 30 days before I need to withdraw. I understand that I am still responsible for that month’s tuition, as a place in class is held for me or my child. I understand that I will still be charged a full month tuition regardless of when during the month I withdraw and give Stars on 9 Dance Center permission to charge that full month to my account. I also understand that tuition, fees, registration fee, costume fees, recital fees and other payments are not refundable nor transferable.

I have read, understand and agree to comply to the recital participation policies as stated in Stars on 9 Dance Center 2023-2024 Handbook and Guidelines. When I choose to order a costume, the deposit is non-refundable and I am then agreeing to participate in the recital. I will pay the costume balance in full. I will receive my (my child’s) costume after all tuition, fees and balances are paid in full. I will pay the non-refundable family recital fee. I (my child) will attend the mandatory rehearsal. I understand that tuition, fees, registration fee, costume fees, recital fees and other payments are not refundable nor transferable.

I hereby authorize Stars on 9 Dance Center to charge my credit/debit card account, on a recurring monthly cycle, beginning on the date I have registered for classes and enrolled as a member of Stars on 9 Dance Center. I understand that autopay process will be automatically electronically debited to my account on the 23rd of the month prior to attending classes and will continue for 9 additional monthly cycles until June 30, 2024. I understand that a 4% processing fee is added to all transactions. I understand that Stars on 9 Dance Center will notify me of the costume balance and other charges and when my account will be charged these fees. This authorization is to remain in full force and effect until Stars on 9 Dance Center has received written notification from me of its termination.

I, AS A PARTICIPANT OR PARENT/GUARDIAN OF SAID CHILD(REN) HEREBY ASSUME ALL OF THE RISKS OF MYSELF/MY CHILD(REN) PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THESE VIRTUAL DANCE/MUSIC CLASSES, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective reigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of myself, my child(ren), for my/my child(ren)’s death, disability, personal injury, or actions of any kind which may hereafter occur to myself/my child(ren) during VIRTUAL DANCE/MUSIC/YOGA/MUSIC CLASSES BY THE FOLLOWING ENTITIES OR PERSONS: Stars on 9 Dance Center and/or their directors, employees, or volunteers. (B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE THE FOLLOWING ENTITIES OR PERSONS: Stars on 9 Dance Center and/or their directors, employees, or volunteers from any and all liabilities or claims made as a result of participation in VIRTUAL DANCE/MUSIC/ CLASSES, whether caused by the negligence of release or otherwise. I acknowledge that Stars on 9 Dance Center and their directors, employees, and volunteers are NOT responsible for errors, omissions, acts, or failures to act of any party or entity conducting VIRTUAL DANCE/MUSIC/YOGA/MUSIC CLASSES on their behalf. I acknowledge that this activity may involve injury. The risks include, but are not limited to, those caused by jumping, leaping, turning, stretching, or any DANCE / MUSIC / YOGA / MUSIC movement, lack of hydration, improper DANCE/MUSIC/YOGA/MUSIC floor, and actions of other people including, but not limited to, participants, and/or producers of the activity. I hereby consent for myself / my child to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during VIRTUAL DANCE CLASSES. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT. I SIGN IT OF MY OWN FREE WILL. I certify that I/my child(ren) are/is physically fit, has sufficiently prepared or trained for participation in VIRTUAL DANCE CLASSES, and has not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my child(ren)’s participation in VIRTUAL DANCE CLASSES. I acknowledge that this Accident Waiver and Release of Liability Form will be used by Stars on 9 Dance Center, the director, class holders, sponsors, and organizers of the activity in which my child may participate, and that it will govern my child(ren)’s actions & responsibilities at VIRTUAL DANCE CLASSES. I permit my child to participate in VIRTUAL DANCE CLASSES.

I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event. In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

I HEREBY WAIVE, RELEASE, AND DISCHARGE Stars on 9 Dance Center and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives, private instruction, choreography or any activity I may choose to participate.

I agree to hold harmless Stars on 9 Dance Center & Fortier Photography, it’s teachers, staff and special guests of any injuries that I, my child, my family members may sustain while attending/ observing dance classes, rehearsals and performances on or off premises. This agreement includes any staff or workers of any outside facilities at which performances or rehearsals may be held. I understand that photos and videos are often taken during classes, rehearsals, and performances. I understand that these photos and videos remain the sole property of both Stars on 9 Dance Center and Fortier Photography. I give Stars on 9 Dance Center & Fortier Photography permission to use my or my child’s (children’s) photo(s) and/or video footage for marketing purposes including social media. I acknowledge that I have read, understand, and agree to comply to the policies of Stars on 9 Dance Center as stated in the 2023-2024 Handbook and Guidelines listed in the Student File Folder in this Family Portal.
