COVID-19 Waiver

Made Talents is following a strict social distancing and cleaning protocol. As every other place right now, we cannot guarantee that you or your child will not be exposed to COVID-19 in our group or private dance classes. By signing/clicking below you agree to waive any claims of liability against Made Talents, including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future if you/your child contracts COVID-19.
Additionally, if I (or my child) feels any symptoms I will not come to the studio and stay home. I also agree to notify Made Talents if myself, my child, or any member of my household contracts or comes into contact with anyone who is positive for COVID-19. This action is to ensure the health and well-being of our dancers and community.
