Assumption of the Risk and Waiver of Liability Relating to COVID-19

The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is highly contagious and is believed to be spread primarily from person-to-person contact. As a result, federal, state, and local governments, along with federal and state health agencies recommend social distancing and have limited the congregation of large groups of people in many locations.
Heart & Sole Dance Studio has put in place preventative measures to reduce the spread of COVID-19 however, Heart & Sole Dance Studio cannot guarantee your child/children, or yourself will not become infected with COVID-19. Attending in studio classes could increase your risk and your child/children’s risk of contracting COVID-19.
In order to stay within the CDC guidelines, the following procedures will need to be followed for in person classes.
1. There will be an entrance and an exit to the lobby to maintain a one-way flow of traffic system. Hand sanitizer will be available throughout the facility.
2. Between classes, barres, mats, and any other equipment will be disinfected.
3. When picking up your child, please practice social distancing.
4. Please have your dancer use the restroom before class.
5. Please keep your child home if they feel ill.
6. Upon closing, teachers will sanitize all surfaces.
7. COVID Liability forms need to be signed.
As legal guardian, I hereby verify my signature that I fully understand and accept the terms and conditions listed above. I acknowledge that COVID-19 is contagious and I voluntarily assume the risk that my child/children and I may be exposed to, or infected by COVID-19 by attending Heart & Sole Dance Studio, and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to, or infected by COVID-19 at Heart & Sole Dance Studio may result from my actions, omissions, or negligence of myself and others including, but not limited to, Heart & Sole Dance Studio employees, volunteers, office manager, directors, owners, and program participants and their families. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Heart & Sole Dance Studio, its employees, volunteers, office manager, directors, owners, and representatives whether a COVID-19 infection occurs before, during, or after participation in any Heart & Sole Dance Studio program.
