I am aware and voluntarily agree to assume all of the risks associated with this program, and accept sole responsibility for any injury to my child and/or children, or myself including, but not limited to, personal injury, disability, illness, death, catastrophic injury, paralysis, damage, loss, claim, liability, or expense, of any kind, that I and/or my child/children may experience or incur in connection with my child/children’s attendance at Heart & Sole Dance Studio classes, or participation in Heart & Sole Dance Studio programming. I agree that Heart & Sole Dance Studio and its facility used for instruction, along with their employees, owners, officers, and directors, shall not be liable for any losses or damages occurring as a result of my child’s participation in the dance and/or tumbling program. I further agree not to commence litigation against Heart & Sole Dance Studio along with their employees, volunteers, owners, officers, and directors relating to its program, or any injury or death occurring from my own/my child’s participation in it. As legal guardian, I hereby verify my signature that I fully understand and accept the terms and conditions listed above.
