Assumption of Risk
Please note that by signing this document, you acknowledge that your minor child and/or you will be voluntarily participating in the described supervised, organized activity sponsored by Starz Elite, LLC. You also acknowledge that participating in such activity involves inherent risks that could include, but are not limited to: sprains, bruises, pulled muscles, and broken bones. You are voluntarily giving your permission to participate which indicates acceptance of such risk by your minor child and/or yourself. Therefore, by entering the activity, you release and hold harmless Starz Elite, LLC and its owners, operators, teachers, fellow students, families, and employees for your minor child and yourself and your heirs and successors from any injuries, illnesses, or damages incurred while participating in Starz Elite, LLC's activities. In addition, Starz Elite, LLC is not held responsible for any personal injury or property loss to students, families or guests that may arise in conjunction with your child's and/or your participation in this activity, even if injury or damage was caused in which whole and or part by the negligence or other fault of one or more of the release parties.
Release of Liability
Please note that by signing this document, you acknowledge the element of risk with the described activity, and therefore such activity should not be attempted without knowledgeable persons spotting in well-matted areas.
Payment Policies
Please note that by signing this document, you acknowledge that we offer classes through to May. Once you sign up for a class, it is our policy that any registration fee and the entire tuition be paid in full, regardless of number of classes attended. If a payment by check is returned without being paid, you must pay a $20 returned check fee. Registration fees are non-refundable.
Commercial Use Policies
Please note that by signing this document you acknowledge that you grant permission to Starz Elite, LLC to use your student's and or your audio or visual image in print of digital/electronic form in photographs, audios, videos, television, or other media for advertising/promotions, news coverage, other commercial use for our events or educational activities.
Credit Card/ Bank Account
All accounts must have a credit card or bank account on file unless paid in full.
