COVID-19 Health and Safety Waiver:
Due to the COVID-19 global pandemic, Carolyn’s Dancenter has made significant changes to our operations and studio space to ensure the health and safety of all persons in the Carolyn’s Dancenter community. All operational decisions at Carolyn’s Dancenter will be based on recommendations by The State of New York. Carolyn’s Dancenter will continue to update guidelines based on the most recent information from the Centers for Disease Control and Prevention. Registrants acknowledge that they are voluntarily participating in all Carolyn’s Dancenter activities. As such, by attending the Carolyn’s Dancenter studio, they may risk exposure to COVID-19 and possible infection. Carolyn’s Dancenter is therefore not responsible for any resulting illness. Registrants also agree to communicate any exposure and/or infection of themselves and/or any individuals in their immediate household.
To enroll in any class the Registration Fee along with the First And Last Installment (Sept And June) are due in full. On the 1st of each month from October-May your credit card on file will automatically be charged one installment. All classes will continue virtually/hybrid if deemed necessary for any reason at the same tuition rate.
Each class participates in the end of year recital. Please contact the studio upon registering if you will not be participating in the recital.
The recital costume fee is $99 and due in October. We cannot hold class placement without payment. All lessons are paid for in advance, tuition is due the first of each month via auto pay. After the 7th of each month a $25 late fee is automatically charged to your account. The registration fee, first and last installment are not refundable at any time. Costume fee/payment is not refundable after October 14th. All other tuition is NOT Refundable after November 14th. Refunds are not given to a dancer that discontinues lessons mid month. Dancers must withdraw from a class seven days prior to the start of an upcoming month via email. In the event a withdrawl is made less than seven days prior to the start of a month the entire next month must be paid. There are No Refunds or deductions for lessons missed as Makeup Classes are available. Every class has ONE MAKEUP LESSON built into the season. In the event a second class is cancelled due to weather or you can't attend due to illness, dancers are welcome to take a Makeup Class in any genre of dance that coordinates with their age. Makeup Lessons must be scheduled with our Studio Coordinator and be made up within 3 weeks from the date of absence. Our tuition is seasonal (with several payment plan options) and make up lessons are not offered for studio closings due to holiday breaks. All account balances must be paid in full to participate in ticket sales, picture day, dress rehearsal and the recital. All photo and video taken of my child are property of Carolyn’s Dancenter and maybe used for marketing material, website, social media or other advertising methods. To my knowledge my child does not have any medical problems that would prevent him or her from participating in CAROLYN’S DANCENTER programs. Please contact the studio if your child has any allergies, special needs or physical limitations that we should know about in order to better serve you. I acknowledge CAROLYN’S DANCENTER dress code (see website) and agree to follow it or my child will not be able to participate. To reduce the risk of injury, please consult a physician before beginning this or any other type of exercise program. I am aware that CAROLYN’S DANCENTER takes every precaution to ensure my child’s safety. However, if an accident should occur and I cannot be notified I authorize Carolyn’s Dancenter to seek emergency treatment at my own expense. I will not hold Carolyn’s Dancenter Inc. or any of the staff members liable. I have read and fully understand the above written statements.
