2022-2023 Waiver

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Melody K. Durnbaugh D.B.A. Special Touch Talent Dance.

I/we agree to release and hold harmless Melody K. Durnbaugh D.B.A. Special Touch Talent Dance, including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Melody K. Durnbaugh D.B.A. Special Touch Talent Dance liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules displayed on the Special Touch Talent Dance website and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Melody K. Durnbaugh D.B.A. Special Touch Talent Dance.

I understand that Melody K. Durnbaugh D.B.A. Special Touch Talent Dance is an insured organization. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Studio Director, instructor or staff member as soon as possible.

I understand the precautionary measures that Special Touch Talent Dance is taking to protect my child from Covid-19 and agree to follow all protocols including but not limited to wearing a proper face covering while entering and exiting the studio, following social distancing guidelines and allowing any person entering the facility to have their temperature taken. I also understand that while these precautionary measures are being taken, there may still be risk for exposure while at Special Touch Talent Dance. If for some reason my child becomes infected with Covid-19 or any member of the immediate household, I/we will not hold Melody K. Durnbaugh D.B.A. Special Touch Talent Dance liable for any personal injury.

I understand the following:
Billing and communication is done via email
All fees for tuition, costuming or recital are non-refundable
If I choose to enroll in auto draft, it will draft my chosen account on the 5th of each month
If for any reason we discontinue lessons at Special Touch Talent, I have to drop them online through the parent portal in order to stop auto draft
