I grant permission to Rising Starz Dance Studio to photograph and/or record my child. I understand that images and videos will be used for informational and promotional purposes for the studio and will not be used for any other purpose. I understand that my child’s picture(s) or video(s) may be shared numerous times on social media, I have not nor will I seek compensation or legal action for the use of my child’s photographs or videos.
