Enrollment Agreement 2024-2025
Billing Information: A guaranteed credit card is required for all registrations regardless of initial payment method. NO EXCEPTIONS.
Tuition will be billed to your card on file on the 1st of each month August 2024 through June 2025 for the upcoming month of class(es). Classes run August 5th, 2024 – June 15, 2025. Summer (camp and/or Intensive) will be billed in the first of July 2025. Parents must inform office if dancer will not be able to attend Summer (camp and/or intensive) 2 weeks prior to camp and/or intensive start day. Schedule for camp and/or intensive are subject to change at studios convenience for student registration.
1. Tuition and Payment Agreement: I agree to pay La Danse Company for the dance instruction of the above student per the published rates and fees for the student's annual registration, each period. La Danse Company, I understand that I can make payments by debit or credit card. I understand that tuition fees are due on the first of each month or first Tuesday of each month; that if payment has not been received or accepted by the 5th of the month, a $15 late charge will be accrued. Please assure the credit card on file is up to date. It is parent’s responsibility keep payment information up to date on file on my DSP APP. Late payments for non-updated information will result in a $15 charge after the 5th of the month. No refunds No exceptions. Monthly statements are issued on you DSP APP. La Danse Company will not make refunds No exceptions. If you want to cancel any tuition, you must make a formal request in the office or email. We will then stop your tuition the first of the following month. La Danse Company resides in the Sweetwater and Chula Vista School District; hence, La Danse Company will follow the school calendar and adhere to all bad weather and holiday school closure due to vacation or health orders or weather alerts for the safety of our staff and dancers. There are no reductions or refunds for vacation, holidays, or school closures No refunds No exceptions. La Danse Company will close 2 weeks after the Winter Recital December 17, 2025, and Summer June 15, 2025. These dates will be posted in studio, social media, and website & email 10 days prior. I understand it is me responsibly to be informed of my dancer’s event, classes, and schedule changes. Tuition is not adjusted for holidays or absences. La Danse Company calendar schedule regarding Holiday: You are responsible for payment for all classes that you are enrolled in, even if absent, until La Danse Company receives a two (2) week notice of withdrawal prior to the end of the month. There is a $65 USD fee for contract cancellation. Missed lessons cannot be used in place of payment; no adjustment or credit will be given for missed classes. Any changes from monthly tuition enrollment either in number of classes, classes enrolled, or changes in styles, shall also be made by EMIAL at least 2-weeks prior to the beginning of the month. I understand tuition is Auto Billed to the credit card on file on the 1st-5th, of each month for the upcoming month of classes. If tuition is not received by the 10th of the month your dancer will not be permitted to participate in their scheduled classes until payment has been received.
2. Summer Camp/ Intensive: (1-2 weeks in July 2025) no regular schedule classes. If your dancer interested on attending, you must notify the office at least 2 weeks in advanced. Schedule and pricing may vary/change at studio and teachers availably. Refunds No Exceptions.
1A) Private locker availability and items of value: Lockers are available for COMPETITION TEAM DANCERS ONLY due to the number of hours of classes. combinations must be given to the studio manager in case lock keys or combinations are forgotten. If La Danse Company believes there is anything that is not allowed stored inside students or for any random inspection. La Danse Company has the right to open lockers and check for these items (parents will be notified before). Lockers will be cut off a week after recital and all items remaining will be donated or go directly to trash. Please keep your lockers clean and sanitized. Students should not bring any personal items of value into out premises or at any La Danse Company event (outside premises) La Danse Company or La Danse Company staff is not responsible for any lost or stolen item. No Refunds No Exceptions.
2. Code of Conduct/ Uniform policy: Appropriate and respectful behavior must be exhibited in our school particularly during class and rehearsals. This includes no side conversations and no cell phone and/or electronic device usage in the classroom. Instructor reserves the right to dismiss the student from class due to misconduct and no refund shall be given. Upon entering class students must wear appropriate uniform and attire, shoes and hair required for each style. This is for students to move and perform to the best of their ability in class. Students will be given 3 classes or notices before she/he is asked to step out of class if attire (complete uniform), shoes or hair is causing a distraction or obstructing the dancer or teacher to view students' posture for safety. La Danse Company may require a different uniform or attire for each class.

- Hip Hop (LOGO t-shirt or sweater, black leggings, or grey sweatpants)
- Contemporary (LOGO shirt or sweater, black or grey sweatpants, black 100% cotton sock)
Any pre-ordered item MUST be purchased in front desk by parent or guardian. Special event or items will have a pre-ordered deadline. Keep updated by mail and/or Instagram.
3.Tardiness /drop off and pick up: Dancers are expected to be on time for class with hair done & shoes on when they enter the classroom. Important warm-ups are missed if a student is late, and injuries could result. If a student is more than 10 minutes late, the student must ask permission to join the class and may sit and observe the rest of the class but will not be allowed to take class. Students are expected to be ready for their lessons. This means a student is expected to arrive, warm up and stretch at least 10 to 15 minutes before a lesson or rehearsal begins. This transition period assists in one focus, physical well-being, and injury prevention. Drop off and pick up must be in 10 minutes waiting period. Students may be dropped off only 5 min before class and must wait in the student area, parents are not allowed in the student area or changing rooms. Students will be released 5 min for quick pick up. Students must be picked up no more than 10 min late. If dancers is picked up later than 10 minutes late after 2 late pick-ups, there will be a late fee pick up $30 USD for staff overtime. Perents must notify the front desk and summit in writing if a student is picked up or carpooled by someone who is not a legal parent or guardian.
4. Recital & Costume: La Danse Company presents a Winter December & Summer June recital. Dancers are required to participate in all recitals for each class style attended. In the case that the dancer may not attend class both training days, La Danse Company does not GUARANTEE their spot in the performance. Dancers may not miss 2 classes in a row OR 4 classes of a given discipline to perform in the recital. Please, submit BY EMAIL, 3 months in advance, any potential conflict for recital date performance for staff review. Parents will remain responsible for any remaining balance of the costumes or additional rehearsal fees. There are No refunds for tuition or costumes. Each dance style requires a separate costume per performance. Costume fees may vary; dancers may require certain style shoes and/or colors all information will be available at the studio. Costumes and sizes are carefully selected according to each company’s size charts. Costumes are not custom fit and may require slight alterations. Parent is responsible for adjustments and alteration. The studio may provide a seamstress for your convenience at your cost. Studio provides a calendar with important events and information by email or in person directly with office teachers also provide verbal information to students in each of their classes and. It is parents or guardian’s responsibility to be aware of events or changes posted in the studio calendar. Recital costume deposit: The amount of $60 will be charged to your account for December recital costume deposit this deposit by October 1st, 2024, for together with your monthly fee. June recital costume deposit will be charged by March 25, 2025. Important information about the recital, event and more will be forwarded to the email provided above in your release enrollment agreement. Please keep email updated. We must receive a NO-PARTICIPATION 2 weeks prior to October 1, 2024 & March 25, 2025, email or office visit for your dancer’s costume not to be charges to your account. No refunds No exceptions.
5. Classroom protocol: Parents are not allowed in classrooms at any time. Knocking, opening doors, or stepping into class is unacceptable if you are not a La Danse Company student. In the case you must speak to a teacher or instructor at any time please make an appointment or notify the front desk. Parents of students under the age of 4 or of students who may need any type of assistance to use the restroom, must remain in the area around the studio in case the student needs assistance. If there is a sibling who may take them to the restroom, please notify the front desk. Parents may not step in the classroom to take their student to the restroom. Students will be guided to the door for you to assist them. Student level is determined by teacher criterion, dancers experience and capability. It is ok for dancers to remain in the same level, class advances each year as a group and teachers determines advances individually, classes are multi-level and students should always work to the best of their capacity. Parent waiting is outside studio front door. Parents are permitted in lobby for payment, merch shopping or general information. There are no parents waiting area inside the studio classroom or office. For more information and levels make an appointment with the teacher or director at the front desk. Parents with any questions about class placements, concerns etc. Must make an appointment prior to speaking to a staff or teacher during office hours from Monday – Thursday (4-7:30) ONLY.
6. Publicity Release: I hereby authorize La Danse Company to record a student's picture and voice on photographs, films, and tapes and to edit these recordings at its discretion. I also give my permission to La Danse Company to use these materials for publicity, advertising/ social media, and sales promotion. I acknowledge that no promise of compensation is made by La Danse Company for such use.
7. Medical Release: In case parent cannot be reached in class or event, I hereby give my permission to the studio management, faculty, staff and chaperones of La Danse Company to authorize emergency medical care that may be required for the above student during participation in class, performances, or any related La Danse Company. This authorization extends throughout the current academic year (2024-2025) and summer, or until the student is no longer enrolled at La Danse Company., whichever comes first.
I have read this registration agreement with my enrollment, and I will carefully read the information and follow the guidelines. I understand and agree with all the information given to me in this enrollment agreement for season 2024-2025.
Waiver and Release from Liability For 2024-2025
My child will participate in dance instruction given by La Danse Company. I acknowledge that I understand the nature of the activities my child will be participating in and the possibility that despite precautions, accidents and/or physical injury may occur. Medical Emergency The undersigned gives permission to La Danse Company, it’s owners and operators, instructor, or assistants to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I recognize the risks of illness and/or injury inherent in any dance/theater exercise program. I hereby declare my dancers prepared physically, mentally and has no restrictions, or condition and/or declare the participant to be in good physical and mental health for participating in any LDC activity.
I/we agree to release and hold harmless the La Danse Company including its teachers, assistant, and staff members from any cause of action, claims, or demands now and in the future. I/we will not hold by La Danse Company liable for any personal injury or any personal property damage or loss, which may occur on the premises before, during or after classes or events which may occur at any La Danse Company or facilities use by La Danse Company or sponsored event outside the studio. I am responsible for me and my child’s actions liable for any personal injury or any personal property damage or loss, which may occur on the premises before, during or after classes or events which may occur at any La Danse Company or facilities use by La Danse Company or sponsored event outside the studio. I take full responsibility for me and my child’s actions where any incident, accident or action that may result in claims, or demands now and in the future by La Danse Company or facilities use by La Danse Company.
Furthermore I/we agree to read and follow the class and facility policies and take full responsibility for my/our (my dancer) behavior in addition to any damage I/we (my dancer) may cause to the facilities utilized by La Danse Company. I take full responsibility for mine and my dancer’s actions during any event, class or activity organized by La Danse Company. I authorize and agree that La Danse Company may take and use photographs, videos or likenesses of myself, or my child as needed for its record keeping, advertising, social media and/or public relations projects and that I have no rights to the same and will not be compensated for the same.
My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation.
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT. In consideration of being allowed to participate on behalf of La Danse Company dance program and related events and activities, the undersigned acknowledges, appreciates, and agrees that: 1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, 2.I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others and assume full responsibility for my participation; and 3.I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest studio director or manager immediately; and, 4.I, for myself and on behalf of my heirs, assigns, personal representatives and next of kind, HEREBY RELEASE AND HOLD HARMLESS (insert name of sports organization) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF PARTICIPATION) * This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. La Danse Company will notify any cases that arise in studio for parents to make the best decision for their dancer. We ask that you have the personal responsibly to inform La Danse Company of any case of infectious or contagious disease, we can inform our staff, parents and students about any changes in or I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releases and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releases for any and all liabilities incident to my minor. Our (parent & dancer) presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE to the fullest extent provided by law. I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND HAVE READ AND UNDERSTOOD PRIOR TO REGISTERING, SIGNING AND BRINGING MY DANCER TO CLASS.
