Waiver of Liability

I am either registering myself as an adult or am the legal guardian of named child(ren) and give permission for said children to fully participate in Mutual Arts activities. I fully recognize the inherent physical risks involved in dancing, movement, crafts, games, and other potential activities, including potential outdoor activities. I do hereby release, forever discharge, and agree to hold harmless Mutual Dance Theatre and Arts Centers, their trustees, staff, instructors, volunteers, employees, and agents and their heirs (collectively known as the “Indemnities”) from and against any and all liability and expenses of any kind that my assignees and representatives may have now or in the future including attorney fees, property damage, personal injury, in association with, or in relation to participation in said camp/class.
