Acceptance of Waiver and Policies
By checking the box below and completing this registration, I acknowledge that I have read, understood, and agree to the following terms and conditions:

PHOTOGRAPHY RELEASE:
I permit the Academy of Ballet, Inc. to use photos of my child at performances, in the studio, or in class at the Academy of Ballet on the Academy of Ballet website/social media, advertisements, and for marketing purposes. I understand that I will not be compensated with money or any other form of payment, credit, or special privilege.

WITHDRAWALS:
All withdrawals must be in writing. Refunds will only be given for remaining classes and are subject to transaction and credit card fees deducted from your payment.
Seasonal Students Withdrawal (August-May in Recital):
Withdrawals after March 31st incur full tuition for April and May.
To stop future payments, withdrawals must be in writing 30 days before the next auto draft.







RELEASE:
I understand that my child will participate in an active program and accept the risks. I acknowledge that I have been informed that this program is not a licensed childcare facility. I also understand that the Georgia Department of Early Childcare and Learning does not require licensing and that this program is exempt from state licensure requirements.

THE CHILDREN’S NUTCRACKER 2024 (All Students ages 2.5 years and up):
Auditions: September 21, 2024, for ages three and up. Audition fee: $25-$35.
Rehearsals: Saturdays beginning September 28, 2024. Mandatory studio rehearsals are held two weeks before the performance. The participation Fee Is TBD.
TBA Performance Date: this is an entire weekend.
Mandatory for Ballet Ensemble Dancers.

TUITION AND REGISTRATION FEES:
Sessions (12 and 5 Week Sessions ONLY):
Tuition and registration fees are due in full at the time of registration. We will send you an invoice from aob.billing@gmail.com.




Prorate for late starts or early withdrawals. For early withdrawals, your refund will have the credit card and transaction fees deducted from your payment.


Seasonal Students (August-May in the Recital. NOT for Sessions):
Agreement: I accept responsibility for payments as per the tuition schedule.
Structure: Tuition is a seasonal/annual fee, paid in full or in 10 installments. Late starts are prorated.
Withdrawal Policy: Withdrawals after March 31st incur full tuition for April and May. To stop future payments, withdrawals must be in writing 30 days before the next auto draft.
Make-up Classes: Available within one month of a missed class—no refunds for missed classes.
Holidays/Weather: Not considered missed classes.

Payment Setup: (NOT for Sessions)
Auto draft: For tuition, costume, and recital fees. Options: credit card, checking, or savings account.
I authorize the Academy of Ballet Inc. to charge my account through Wave accounting systems.
NSF Payments: $35 fee for each NSF. After two NSF payments, a $60 fee, and future payments in cash by appointment.
Late Payments: Payments after the 10th incur a $15 late fee.

AOB SPRING RECITAL 2025 (NOT for Sessions):
Recital Date: TBA (classes continue through May 2025).
Participation Requirement: Pay tuition through April and May.
Recital Fee: $185 per dancer or $314.50 per family. Includes space rental, production costs, payroll, and other expenses. This fee will be included in auto-drafted monthly installments, total due by May 1, 2025.
Tickets: Dancers will receive 4 tickets per performer or 6 tickets per family to the performance or performances that they are performing in. These tickets cannot be exchanged for different shows. Extra tickets can be purchased online.
Non-Participation Notification: Inform us ASAP or by October 31st to receive a refund or credit for the recital fee included in your installments.

COSTUME FEES (NOT for Sessions):
Cost: $115 and up per costume.
Payment: Not included in auto-drafted installments. Full payment is due by November 15th, 2024.

4. The Children’s Nutcracker 2024: I understand the audition and rehearsal requirements and fees for participation in The Children’s Nutcracker 2024.
5. Tuition and Registration Fees: I agree to pay all tuition and registration fees in full at the time of registration. I understand that fees are prorated for late starts or early withdrawals, and refunds for early withdrawals will have transaction and credit card fees deducted.




6. Payment Setup: I authorize the Academy of Ballet, Inc. to charge my account for tuition, costume, and recital fees through Wave accounting systems. I acknowledge that NSF and late payments will incur additional fees as specified.
7. AOB Spring Recital 2025: I understand the participation requirements, recital fees, and deadlines related to the AOB Spring Recital 2025. I agree to inform the Academy of Ballet by October 31st if my child will not participate in order to receive a refund or credit for the recital fee.
8. Costume Fees: I agree to pay the costume fees in full by November 15th, 2024, as these fees are not included in auto-drafted installments.
9. Recital Tickets: I understand that each dancer will receive 4 tickets per performer or 6 tickets per family to the performance or performances they are participating in. These tickets cannot be exchanged for different shows. Extra tickets can be purchased online.
I understand that by completing this registration, I agree to all the above terms and conditions.
