Liability Release
I am aware that dance training and the athletic exercise associated with it places unusual stress on the body and carries a risk of physical injury.
I knowingly agree that I/minor are in good physical condition and hold ODAT and ODRC harmless if injury occurs.
On behalf of myself, or on behalf of the minor for whom I am a legal guardian, I assume the risk and agree that ODAT and/or ODRC shall not be liable in any way for injuries sustained during attendance at dance classes, theatrical rehearsals, performances, or related functions.
I understand that good dance training and/or rehearsing involves touching and adjustment of the student’s body by the instructor, coach, director, or choreographer.
I further acknowledge that ODAT and/or ODRC and their instructors are not responsible for any loss of, or damage to, the student’s personal property.
Medical Release
In the event that I cannot be reached, I hereby give my permission to the management, faculty, staff, and chaperones of ODAT and/or ODRC, to authorize any emergency medical care that may be required for myself or, on behalf of the minor for whom I am a legal guardian, during the participation of classes, rehearsals, performances, or a related ODAT and/or ODRC event. I understand that I am responsible for any and all charges as a result of such care or medical treatment.
I understand the if I, or the minor for whom I am a legal guardian, requires administration of any special medical care, a doctor's note must be provided with detailed instructions regarding the administration of any medications for the following, such as:
Asthma - inhaler(s)
Allergic Reactions - epinephrine pen
Diabetes - insulin or special medication
Epilepsy - protocol if a seizure happens and medication list must be on file
Hypoglycemia - provided snacks or specific medication
Publicity Release
I, the undersigned, or on behalf of the minor for whom I am a legal guardian, do hereby grant permission to ODAT and/or ODRC to publish stories, photos, videos, or other items related to any of my experience or appearances, hereinafter referred to as “Materials,” on any ODAT and/or ODRC website(s) and all social media platforms, as well as print, TV, radio, or podcast media. I hereby release you, your representatives, employees, managers, members, officers, Board of Directors, related entities, and subsidiaries, including without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation, and any other personal and/or property. I acknowledge and agree that no sums whatsoever will be due to me as a result of the use and/or exploitation of the Materials or any therein. I also agree that ODAT and ODRC retain sole ownership of Materials in perpetuity.
The undersigned understands and agrees that Ovations Dance Repertory Company and/or Ovations Dance Academy of Tampa reserves the right to void the registration form and participation agreements for contacts like contravene the objective, rules, regulations and policies of any performance productions, ODRC and Ovations Dance Academy of Tampa.
