2021-2022 FLDS Financial Agreement
Please read and initial each of the following terms.
__________ Tuition Fees are due on the 1st of each month, and will be processed through my credit card on file. My Credit card will be charged a 1.5% convenience fee for each transaction made. This fee goes directly to the credit card merchant.
__________ Studio Fees are due on the 15th of each month and will be processed through my card on file. My Credit card will be charged a 1.5% convenience fee for each transaction made. This fee goes directly to the credit card merchant.
__________ There will be no refunds or prorating for team practices missed through the entire season
__________ Additional practices may be added prior to major competitions; no additional charges for these hours
__________ Practices may be canceled due to holidays, competitions and inclement weather
__________ Athletes will NOT be allowed to participate in practices or competitions until balance is current on delinquent accounts
__________ Up-front payment receives a 10% discount for Studio Fees only, if made in full during the first month of registration.
_________ Extended travel teams may have additional competition and coach’s fees for extended travel competitions, including but not limited to bid competitions, Dance Worlds, Dance Summit and the US finals.
__________ We accept, cash, cash app, Pay Pal and Venmo.
__________ We DO NOT accept checks!
__________ I will register my athlete on the FLDS Parent Portal.
