New student registration waiver

INSURANCE WAIVER: The undersigned, on behalf of the student, hereby release North Country School of Dance, directors and any instructor, associate, or volunteer from any or all damages or injuries including illness which we may sustain while participating in any activity, class, performance, competition, (in or out of the state of Vermont) connected with this school of dance. All Classes and events will be held in compliance with the CDC and State and Local regulations. We also authorize NCSD the use of their photos or videos for advertising purposes in newspapers and the internet.
