POLICIES AND WAIVER
ENROLLMENT: Enrollment in a class or classes requires full payment, and each month, only the Initial Tuition fee will be prorated when signed up during the month. Dazzle Dance Studios reserves the right to cancel a class due to insufficient enrollment and will offer an alternate class to attend.
TUITION: Tuition is paid monthly to hold a student's place in class or classes and must be paid regardless of the student's attendance each month. Some months will have five weeks of class, while others will have less; however, monthly tuition will remain the same. Tuition is due by the Second of each month and considered past due after the 10th; at that time, a $15.00 late fee is added to your account. If tuition becomes two months delinquent, students are subject to removal from classes or informing Dazzle Dance Studios of an absence of time—NO REFUNDS on any Tuition.
MISSED CLASSES: We only offer make-up classes for missed classes of the same class, made up by the following month. No reductions or refunds are given for missed classes.
PAYMENTS: Please have a credit card on file to be debited monthly for tuition. Other fees, such as dress code, costumes, or competition wear, will be charged to your card on file, with notice of charge taken. unless notice is given with another form of payment or non-participation in the Recital.
HOLIDAYS: Please remember, there is NO prorating for the Holiday Months. Dates are subject to change.
LOST AND FOUND: Items lost at the studio are kept for 30 days only. Please check the office regularly, as items are donated monthly. DDS will not be held responsible for lost or stolen items.
SAFETY: I hereby release and hold harmless Dazzle Dance Studios, Inc., its owners, and all employees from all liabilities, suits, claims, and/or demands of any kind or nature, legal or financial, whether caused in any way by negligence or participation in any activities. The student registered voluntarily participates in Dazzle Dance Studios Inc., and I understand that certain risks are inherent to and from participation and involvement in various activities. Parents or guardians must be responsible for medical insurance for their students.
DRESS CODE: A dress code must be worn in each class. DDS strongly suggests that the dress code is adhered to. This is for your dancer's benefit. With proper clothes, it is easier to critique movement and style.
WITHDRAWALS: Students are registered continually month-to-month unless we are given 30 days written notice. You will be charged your monthly tuition rate if notice is not received.

RECITAL CLASS COSTUMES: Each student is measured in November for their costume. All costumes are ordered in December to ensure delivery before the end of April for necessary fittings and alterations. Costumes ordered after December will incur a late fee. (Costume Price includes: Costume, one pair of tights or socks, & accessories)
NO REFUNDS will be given on Costumes, and costumes must be picked up within 30 days, or they will become the property of Dazzle Dance Studios.
RECITAL PICTURES If not picked up at the Recital, they will be held at the studio for 30 days; after that, they will become the property of Dazzle Dance Studios.
PHOTO RELEASE: I am aware that DDS staff members may take photographs and videos of my child/children. I give my permission for DDS to use such photographs and videos for promotional and advertising purposes.
I have read and agreed to the Policies and Waivers for Dazzle Dance Studios, Inc.
I agree to their policies and waiver by signing my child up for class/classes.

We do not have Annual Registration Fees. The charge (Labeled Registration Fees) will be applied to your costume payment.
POM POM FEES: $25.00 for your Pom/Cheer class (If Selected),
PARADE FEES: 100.00 (includes costume fee) If you choose to dance in the Wellington Parade in December
OUR SEASON starts In August and goes to the Recital, in June we never stop dancing all summer.
