Authorization and Release
At Centre Stage Dance Studio, we prioritize the safety and well-being of all our students. To ensure a positive and secure learning environment, we require all participants to read and sign the following waiver:
1. Assumption of Risk: I understand and acknowledge that participation in dance classes, workshops, performances, and related activities involves inherent risks, including but not limited to physical injury, emotional distress, and property damage. I voluntarily assume all such risks and waive any claims, under any theory whatsoever, against Tylene Nichols LCC; DBA Centre Stage Dance Studio Oro Valley, Centre Stage Dance Studio, its owners, instructors, staff, and affiliates (individually and/or collectively “CSDS”) for any injuries, losses, or damages arising from my participation in dance activities.
2. Personal Responsibility: I agree to abide by all safety guidelines, rules, and instructions included in this document and/or provided by CSDS’s instructors and staff during dance classes and events. I understand that failure to comply with these guidelines may result in my removal from the premises and the forfeiture of any fees paid.
3. Release of Liability: In consideration of being permitted to participate in dance activities at CSDS, I hereby release, waive, discharge, and covenant not to sue CSDS, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, while participating in dance classes or events.
4. Medical Authorization: I authorize CSDS to seek medical treatment on my behalf in the event of an emergency or injury occurring during dance classes or events, if deemed necessary. I voluntarily consent to receive emergency medical care, including but not limited to first aid, CPR, administration of medication, and transportation to a medical facility, if deemed necessary by the staff or authorized personnel. I understand that every effort will be made to contact emergency contacts listed on file before initiating any medical treatment. I understand that I am responsible for any medical expenses incurred as a result of such treatment. I release and hold harmless CSDS, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any emergency medical treatment provided to me during my participation in dance activities.
5. Lost/Stolen Items: I understand and acknowledge that while participating in activities organized by CSDS, there is a risk of loss or theft of personal belongings. I hereby release and hold harmless CSDS, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to the loss or theft of my personal belongings. I acknowledge that I am solely responsible for the security and safekeeping of my personal belongings while on the premises of CSDS and that CSDS cannot guarantee the safety of personal belongings. I understand that CSDS is not liable for any lost or stolen items, including but not limited to electronic devices, wallets, jewelry, clothing, or other personal property.
6. Indemnification: In consideration of participating in activities organized by CSDS, the participant agrees to indemnify and hold harmless CSDS, its owners, instructors, employees, and agents from any and all claims, demands, actions, liabilities, damages, losses, costs, and expenses (including reasonable attorney's fees) arising out of or related to: (i) any injury, illness, or harm suffered by the participant or any third party during participation in dance activities organized by CSDS, (ii) any damage to or loss of personal property belonging to the participant or any third party while on the premises of CSDS, (iii) any breach of the participant's representations, warranties, or obligations under the terms of this document, (iv) any violation of laws, regulations, or policies by the participant during participation in dance activities organized by CSDS, and (v) any act or omission of the participant that causes harm, injury, or damage to others or to the property of others.
7. Photos and Videos: I grant CSDS permission to photograph, videotape, and/or record my likeness and/or voice during dance classes, performances, or events for promotional and educational purposes. I understand that these materials may be used in print, online, and social media platforms, and I waive any right to compensation for such use.
8. Tuition: Tuition is per month and is not dependent on the number of classes in the month. Some months there will be fewer classes due to holidays, etc. Tuition is charged to the card on file the first of each month. All lessons must be paid for whether the student is in class or not. If a student registers late in the season, the all-inclusive fees will be added to the monthly tuition to ensure full payment for costumes, recital and registration fees, etc. There is a $25 late fee for tuition paid after the 1st of each month, even if your child has been absent.
9. Emails: Monthly email newsletters are sent out to parents. Please ensure that we have your correct email address. Important information is sent out and it is YOUR responsibility to read these emails and know what is going on. All important information will be sent via email.
10. Absences and Make-up lessons: In case of absence for any reason, please notify the director to set up a make-up class. Make-ups are only available for some classes so please make sure to be in class as much as possible.
11. Refunds/Returns: CSDS does not issue refunds for any reason. CSDS is a professional program for dedicated dancers. In order to drop all classes and be removed from Auto Pay, a 14-day notice must be given to the studio. Auto Pay tuition will still be run on the 1st of each month unless a request to drop has been processed 14 days prior to the 1st of the following month. Notice can be given by contacting the studio directly with a written notice to drop classes. This written notice must be submitted by email to centrestageov@gmail.com
12. Autopay: I agree to have my card on file charged according to the tuition, private lessons (if applicable) and charge schedule for which my child is enrolled. I recognize and agree to no refunds. I will make sure that my credit card on file is up to date. There will be a $25 late fee on all charges that are not paid by the first of each month.
13. CSDS’s Protocol for Students/Parents:
-Parents should not leave until their child has been admitted into the building.
-Parents are to wait outside during class times to reduce distractions in the studio.
-Students/Parents will exhibit appropriate behavior in and out of the studio and towards CSDS staff and other students
-Students/Parents must do their best to be on time and perpetually tardy students will not be allowed to continue.
-Students that show symptoms or have a fever over 100 will not be admitted.
-At all times, please do not send your child to class if she or someone in your household is sick or has been sick in the last 14 days. Please take advantage of our virtual options when someone in the house is sick.
By signing this waiver, I acknowledge that I have read, understood, and voluntarily agree to all terms and conditions outlined herein. I understand that this waiver is legally binding and remains in effect for all present and future dance activities at CSDS.
