Ozark Dance Center Enrollment and Tuition Policies
Ozark Dance Center is dedicated to artistic and competitive performance. Ozark Dance center is dedicated
to providing quality dance instruction to all of our students. We want to let children be children in a safe
environment where parents do not have to worry about what their children are being taught or exposed to.
We will place a high priority on self worth, modest costumes, fun music without explicit lyrics and age
appropriate choreography. Our goal is to create a fun, artistic atmosphere. At the completion of
their education at Ozark Dance Center the dancer should have gained a higher degree of skill in
dance technique, choreography, and dance performance.
1. MONTHLY PAYMENT IS REQUIRED BEFORE ANY SERVICE IS PROVIDED .
a. Ozark Dance Center requires monthly payments of $45.00 per month or a Flat fee
of $540.00 for total 12 months tuition. If you prefer to pay in seasons. Fall season
is a Flat fee of $405.00 for a total of 9 months or Summer season is a Flat fee of
$90.00 for a total of 2 months (costume fees are included, dance shoes are
extra). Payments must be paid before any service is provided. If an account is
past due, the student may not attend classes until the account is made current. Full
payment is required regardless of a student’s absence from attending class.
b. This tuition covers the full year regardless of how many weeks your dancer
dances. I have taken into consideration that some months may be 5 classes and
months like December, will only be 2 classes.
c. I understand my child’s enrollment is for the entire session of Ozark Dance
Center. By registering, I accept responsibility for the total tuition regardless of
attendance.
d. Trade in Dance Shoe- Buy your first pair of dance shoes. Once your child outgrows the
shoes and they no longer fit, the Dancer is allowed to Trade in shoes for a free pair of
shoes that do fit. Shoes must be in good condition in order to “trade in”
.
2. REQUIRED MATERIALS:
The dancer should treat each class as a rehearsal by giving his/her best effort. All shoes needed
for all dancers should be brought to every class practice. Attire will consist of comfortable but modest
dance basics. Gum or Jewelry is not allowed.
3. BEHAVIOR:
Ozark Dance Center members are required to adhere to all the rules of the contract. In class,
dancers are required to follow common rules of courtesy, listen to and follow directions, be respectful of
him/herself and others, be positive in their attitude about learning and comments to peers, be prepared,
and participate in all dances.
4. PERFORMANCES
All dancers are required to perform at Recitals, unless I have one month advance notice in order
to prepare and make arrangements for the class/ Recital.
All Ozark Dance Center Members are held accountable to the aforementioned guidelines of the
Enrollment and Tuition Policy. Please sign below to acknowledge that the student and parent

Photo Consent Form
As a parent or guardian of this student, I hereby consent to the use of photography/videotape
taken during the course of Ozark Dance year for publicity, promotional, and/or educational
purposes (including internet or other media sources).
___ Yes, I give consent for Ozark Dance Center to photograph my child for dance purposes
and/or dance events.
___No, I do not authorize Ozark Dance Center to photograph my child for any event.
