ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I hereby assume all of the risks participating in Lisa's School of Dance Inc, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of possible liability without fault.
I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in Lisa's School of Dance Inc dance classes. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of the Lisa's School of Dance Inc in which I may participate and that it will govern my actions and responsibilities at said Lisa's School of Dance dance classes and activities.
In consideration of my application and permitting me to participate in Lisa's School of Dance Inc, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from Lisa's School of Dance Inc. THE FOLLOWING ENTITIES OR PERSONS: ALL associated with Lisa's School of Dance, and/or their coaches, instructors, agents, representatives, or volunteers.
(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in Lisa's School of Dance classes, whether caused by negligence or otherwise.
I acknowledge that Lisa's School of Dance Inc may carry with it the potential for death, serious injury, and personal loss. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, coaches, and lack of hydration.
I consent and agree that Lisa's School of Dance Inc and/or their instructors, coaches, agents, representatives or volunteers may take photographs or digital recordings of me as a participant during this event and use these in any and all media for training or promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims or interest and I understand that there will be no financial or other remuneration.
The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under law.
I have received or will obtain a copy of Lisa's School of Dance policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Lisa's School of Dance does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Lisa's School of Dance, and I voluntarily assume the risks associated with such participation. I further understand and assume the risks associated with participating at dance events, performances, and recitals held off premises and on premises and do not hold Lisa's School of Dance or its employees liable for damages, losses, or injury that may result from my child's participation with Lisa's School of Dance and I voluntarily assume the risks associated with such participation. I understand that a Recital costume must be purchased to participate in the Holiday Showcase and the Annual Recital. Once the orders have been received and processed, there are no refunds or returns on costumes.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL.

Special care will be taken to protect the privacy and security of your information. This policy explains our information practices and the way that Lisa’s School of dance collects and uses your information. This Privacy Policy covers the information you provide to us offline and at the Lisa's School of Dance website.
You can visit our Website and find out about Lisa's School of Dance products and services without giving us any personally identifying information about you. When you visit our Website, we do not collect any personally identifiable information without your consent. We collect and store information from you that you voluntarily choose to give us or enter on our website. When you register for classes at Lisa's School of Dance, we collect contact information such as parent/guardian names, addresses, phone numbers, e-mail address and student information such as names, birthdays, grade, etc. This information is needed to manage studio business and will not be used or sold for any other purpose, except as described below.
Lisa's School of Dance will access your personal information and/or share it with third parties only for the following purposes:
-as needed for studio business
-if required to do so by law or to comply with legal process
-to protect the rights, property, or personal safety of the public.
