PDC Registration/ Participation Waiver

Release of Liability:
I understand that injuries are a natural possibility in athletic endeavors and will assume full responsibility for any injuries incurred at Prestige Dance Company and/or Prestige Elite Athletics. Therefore, I agree to hold Prestige Dance Company and/or Prestige Elite Athletics, the faculty, property owners and all organizations associated with and/or in residence at Prestige Dance Company and/or Prestige Elite Athletics harmless of and from any and all claims, demands, liabilities, losses, lawsuits, judgments, costs, damages and other expenses of any kind or nature whatsoever.
Authorization to contact Emergency Services:
I hereby authorize that, in the event of a medical emergency, Prestige Dance Company and/ or Prestige Elite Athletics is to contact Emergency Services as they deem necessary.
