Policy/Student Handbook

I have received or will obtain a copy of Kelly's School of Dance's policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Kelly's School of Dance does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my child's participation with Kelly's School of Dance, and I voluntarily assume the risks associated with such participation. Kelly's School of Dance reserves the right to withdraw any student from classes or performances who does not comply with the rules or regulations as laid out by Kelly's School of Dance.

I understand and have read the KSD Student handbook. I am aware that it is available to me via the studio website kellysschoolofdance.org. If I do not sign, I am aware that by default I am assuming consent of the handbook.

I understand that a 30-day written notice will be given should we decide not to continue with classes.

In consideration of the benefits derived from Kelly's School of Dance, I (we) do hereby agree to hold harmless Kelly's School of Dance, its teachers, volunteers, agents or employees, from any and all claims for personal injuries to myself, or my (our) boy or girl while participating in said dance, tumbling or any other Kelly's School of Dance activities.

I hereby agree to all the terms, as laid out in the Tuition Policy, I agree to pay all fees as laid out in the tuition policy, the class fee structure, the termination notice, the recital rules and the costume policies.

I hereby, grant Kelly's School of Dance permission to request Emergency Services on behalf of my child, should an emergency situation arise while I am not in attendance.
