Wesrchester Performing Arts Studio LLC. , faculty and staff members will not accept responsibility for any injuries sustained by any dancer during the course of dancing. Common dance injuries that occur at studios are broken ankles, sprained ankles, chins being fallen on and split open, and so on.

Westchester Performing Arts Studio LLC., faculty and staff members will not accept responsibility for any injuries sustained by any dancer during the course of any exhibition, recital, competition, convention or class in which he or she may participate, or while traveling to or from an exhibition, recital, competition, convention or class. All students and parents are required to sign a legal disclaimer. I understand that dancing is a physical activity that carries certain risks and injury. I do hereby agree to hold harmless the Westchester Performing Arts Studio, Ardsley, NY, its Agents, Director or Employees.

I understand that tuition payments are drafted monthly and I will provide credit card information to be kept on file. I hereby grant WPAS permission to use at any time, a photograph or videotape of this student in connection with any WPAS activity. If there is any conflict due to religious beliefs, please let us know. I hereby acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I and/or my child may be exposed to or infected by COVID-19 and other contagious diseases and viruses. I release Westchester Performing Arts Studio LLC, its instructors, independent contractors, volunteers, and all associates from liability for harm or injury pertaining to COVID-19 and other contagious viruses or diseases. I understand that my student must have this release form signed before attending class. Masks are optional for all students in any class.

Tuition is an annual (10 months) rate that can be paid in full, half the year, or in monthly instalments. There are NO PRO-RATED FEES for missed classes or holiday breaks.
