The undersigned individual (the Member), hereby indicates their desire to become a member of Micaiah's Studio of Ballet and LivFit Fitness LLC, pursuant to the terms and conditions of this gym membership agreement.
Payment Policies:
REGISTRATION FEES: One-time registration fee due upon enrollment: $35 (per student) due at the time of registration and is non-refundable. An Electronic Payment Authorization form must be submitted upon enrollment. *VIP & Early Enrollment may have their Registration Fee Waived.
Drop In Rates: $20 a class (must be paid before entering class)
We accept The Art of Wellness Scholarship: This cost share scholarship Application is available at the front desk. This scholarship is provided by the Dare to Create Foundation for the Arts. This cost share program is available to those who qualify and so long there are funds available.
PAYMENT METHODS: We accept Credit Cards, Check or Cash. All dance classes are Set up with Autopay. Autopay payments go out on the 1st of every month.
DUE DATES: Tuition is due on the 1st of each month (tuition is considered late on the 15th), including the months that begin with an observed holiday. A $25 late fee will be added to your account on the 16th of each month.
LATE FEES/DECLINED CREDIT CARDS: Tuition payments that are declined are subject to late fees and any bank fees.
Pauses Classes:
You may pause your classes at any time. Please contact the front desk or email (LivFit4life@outlook.com) for when you wish to pause your membership. You will be allowed a certain amount of time to pause your membership to hold onto your current member rate. If you pause your classes before the 1st of the next month, you will not be charged tuition for the following month. If you pause your classes after the first of the Month, you will still be charged that Month tuition.
TERMINATION AND REFUNDS: Registration fees are non-refundable. Costume and recital Fees are non-refundable. MSB Competitive Company Fees are non-refundable. MSB does not offer refunds for missed classes, including withdrawal from our studio or paused memberships.
Cancelation of Membership: Micaiah's Studio of Ballet & LivFit Fitness must be notified in writing (LivFit4life@outlook.com) when a student will be canceling their classes from MSB before the first of the following Month. If Micaiah's Studio of Ballet & LivFit Fitness is not notified in writing, billing will continue on a monthly basis. Instead of canceling you can also put a pause on your classes. This will allow you to hold onto your current class rate.
Micaiah's Studio of Ballet & LivFit Fitness reserves the right to terminate participation with MSB based on student and/or parent conduct.
ABSENCES AND MAKEUP CLASSES: We do NOT prorate tuition for missed classes- this includes reasons such as: ILLNESS, FAMILY VACATION etc. However, if you miss classes you will have an opportunity to make your missed classes during Make Up Weeks.
Please contact MSB by email or text if your dancer will be absent for classes.
Weather Cancelations/MSB Closings: MSB has the right to close the studio for inclement weather. We follow the Valley City School System protocols for school consolations due to weather. No make-up classes for weather closings as the MSB schedule has built in extra days to cover any inclement weather.
Our Studio: Micaiah's Studio of Ballet & LivFit Fitness is is Dance and Fitness studio. We believe that dance and fitness go hand and hand. We offer a many different programs, from our Pre-School Dance program to Training Creative Dancers program to our Tribe Fitness Program. We believe that Ballet is the foundation for any form of dance as well as gymnastics and ice skating. Dancers who wish to take dance in the genre forms of lyrical and acrobatics are required to take our Training Creative Dancers Classes. Dancers who do not take TCD to compliment these genres of dance have a more difficult time picking up steps and end up getting behind in these classes. Hip Hop and Tap do not require a student to take our Training Creative Dancers class however this will be encouraged.
Any student wishing to compete will need to be taking two or more dance classes a week
POINTE SHOES; Dancers will be considered for pointe by the discretion of the Director. Dancers who are accepted onto pointe are required to adhere by the following rules:
Must be assessed by Micaiah Lasuche
Must have taken 2-4yrs of prior training
Must take 2 OR MORE CLASSES A WEEK. Dancer may be required to continue Foot Focus training (will be discussed with each dancer on an individual basis)
3. Dancer must maintain 2 OR more classes a week to continue on pointe through their training, if dancer drops classes, dancer will be required to stop pointe work for safety purposes.
CODE OF CONDUCT
As courtesy to your instructor and fellow students, we ask that you follow our etiquette guidelines when dancing at Micaiah's Studio of Ballet. Your best behavior before, during, and after class will make you stand out as a serious and committed dance student. These are things to get the most from your time and training at Micaiah's Studio of Ballet.
1. Students must wear the required dance attire to class in order to participate in class. This includes correct hair, uniform leotard, tights, and requisite shoes.
2.Gum, food and drinks are not allowed on the dance floor (a closed water bottle is okay).
3. Come to class prepared to work physically and mentally. If you are allowed in the studio before class, you are expected to be warming up quietly with gentle stretches, not running or jumping around, especially in hallways.
4. Be ready and lined up 5 min. before class starts. Please use the restroom BEFORE class.
5. Please arrive on time, if late you must wait until the instructor allows you in class. If a student arrives more than 15 min late to class, they must do a 5 min warm up before participating in class.
6. Once class has started, you are not allowed to leave the studio unless explicitly given a break by the teacher or in an emergency. All students must ask to use the bathroom.
7. No shoes other than dance or clean athletic shoes on the studio floors.
8. Please stand upright at the barre and anywhere else. Be attentive at all times, especially when waiting for your turn. The mirrors in the classroom are only for helping to correct lines; avoid staring in the mirror while you are dancing.
9. illness injured or ill, students must participate in all combinations in class. Combinations must be completed whether or not you feel its going well. You must never walk off the floor before finishing the step (unless injured).
10. Dancers must treat classmates with kindness and respect. If necessary the Director will address incidents of concern or unkind language with parents in an arranged conference.
11. Do not wear dangling or sharp-edged jewelry.
12. Leave your stuff in the dressing room or locker or in designated areas. You may check for cell phone messages from parents during breaks between classes or at the end of class, not during class.
13. If you must leave early please talk to the teacher before class. If you need to exit in an emergency, exit as quickly and discreetly as possible.
14. Your teacher is to be treated with the utmost respect at all times. Please quietly listen while the teacher is speaking.
15. Listen first, then ask relevant questions. Respect the personal space of others. Respect the dance space. Pick up trash, your clothes, and don’t turn things on, off, up, or down in the space without permission. Watch your language, even when you mess up.
16. Do your homework and practice your choreography at home when asked to do so.
No Gossip Policy: We ask that dancers and parents speak positively about one another or the teachers. Negative discussion about parts in shows/levels is prohibited. We view all dancers and families as part of our big MSB family. Absolutely no gossiping about other studios or companies will be tolerated. We want Micaiah’s Studio of Ballet & LivFit Fitness to be known for supporting the art of dance in our community. We want our language with one another to build each other up and encourage each other to grow into our best self at all times.
Outside of Classroom Behavior:
We expect all members to maintain a positive presence online and offline that encompasses the core values of Micaiah’s Studio of Ballet & LivFit Fitness LLC.
Dress Code: Dancers will follow full dress code details at all times during classes and rehearsal. A dancer out of dress code after given one reminder may be asked to sit out from a rehearsal or class which may count as an absence.
What to Wear to Class
Ballet Clothing - leotard, tights, can be paired with tanks, shorts, and leggings. No Jeans. A leotard and tights can best show off your technique. Helpful if you can see your ankles so we can check foot positioning.
Boys Attire: Black or Blue sweats or athletic pants and Black or White shirt, Studio Shirts are acceptable.
Shoes - ballet shoes for all levels of ballet classes. Can wear socks for the first few classes however need some grip to stop you from slipping (yoga socks are best).
Lyrical Clothes - Comfortable moveable clothing such as leotards, tanks, shorts, leggings. No Jeans.
Shoes - Turn shoes
Tap Clothes - fitness wear or leisure clothes (skirts, dresses, jeans etc.) as most of the movement is from the knee down. However in more advanced levels best to wear something you can move freely in as there will be more full body movements including arms and turns.
Shoes - tap shoes, these can be any make or style whichever you feel most comfortable in. Also think about heel height, you need to be able to do steps on your toes so if the heel is quite high, this will take more strength in your calf muscles to stay on your toes.
Hip Hop Clothes - Comfortable moveable clothing such as tanks, shorts. No Jeans. Clean supportive tennis shoes
Shoes - Clean pair of sneakers only worn to dance, very important note, these sneakers must be worn in studio only, never outside.
Adult & Fitness Classes: Please wear athletic type clothing that makes you feel comfortable and that you can move in. Please wear the appropriate shoes for the class. .
Micaiah's Studio of Ballet & LivFit Fitness has the right to refuse services and dismiss any families if the Owner feels that rules are not being followed
Health Attestation
The Member attests that they are fully able to participate in an exercise regimen of their design or choosing without undue risk.
Gym Rules & Participation
The Member hereby agrees to abide by all posted safety guidelines and regulations while using Micaiah's Studio of Ballet and LivFit Fitness LLC facilities and equipment.
Additionally, the Member agrees to dress and conduct themselves in a manner deemed appropriate for a dance/fitness facility.
The Member shall not consume drugs, alcohol, or tobacco products on Micaiah's Studio of Ballet and LivFit Fitness LLC property.
The Member agrees not to photograph or videotape while in Micaiah's Studio of Ballet and LivFit Fitness LLC bathrooms or dressing rooms.
Micaiah's Studio of Ballet and LivFit Fitness LLC reserves the right to revoke the Members access if these terms are violated
Binding Law
This studio membership contract shall be considered binding upon the Member and Micaiah's Studio of Ballet and LivFit Fitness LLC and shall be upheld and enforced in accordance with the laws of North Dakota. Any legal proceedings related to this Studio membership contract shall take place in courts located in North Dakota.
Acceptance
By clicking on the acceptance Terms parties hereby enter into this Studio membership contract with one another in acknowledgment and acceptance of the terms listed above.
