MEDICAL EMERGENCY
In the event of an emergency, the undersigned gives permission to Zoetic Movement Company, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. In the event of a medical emergency, I hereby consent for Zoetic Movement Company, its owners and operators to activate the emergency medical response (911) and have my participant transported to the nearest available medical facility.
RELEASE OF LIABILITY
As the legal parent or guardian, I release and hold harmless Zoetic Movement Company, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Zoetic Movement Company, its owners and operators or in route to or from any of said premises. The undersigned gives permission to Zoetic Movement Company, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I also acknowledge the risk of exposure to communicable diseases such as influenza, RSV, Covid-19 and other air-borne and droplet dispersed diseases through the activities provided by Zoetic Movement and its operators and willing take the risk for my participant and hold harmless Zoetic Movement from any harm that may result from any infections obtained while in the services of Zoetic Movement and its operators.
ANNUAL REGISTRATION FEE:
Yearly registration fee is $25. If dancer takes more than a month off from dance they must re register and pay the $25 registration again.
PAYMENT:
Tuition is to be paid in full by the 5th of every month through auto-pay. If unable to auto-pay an additional charge of $10 charge towards monthly tuition.
MEDIA RELEASE:
I hereby consent for my participants likeness to be used in the form of photo and/or video at the discretion of Zoetic Movement for the purposes of marketing alone. I understand these photos and/or video will be used and posted in various social media forums, flyers, or any other marketing materials.
FREEZE:
A one-time freeze may be used for all dance families per year. However, a fee of $20 may be applied
CANCELLATIONS:
Cancellations must be communicated in writing: within 14 days of next month’s auto-pay if planning to take vacation/out of town. If planning to quit dance at Zoetic Movement, writing must be madewithin 14 days of next month’s auto-pay.
ROOM RENTAL:
Room rentals are available at $65/ hr, at 2 hr minimum
SCHEDULE CHANGES:
I understand as a nature of business the current schedule is subject to change at the discretion of Zoetic Movement
