Registration Fee $25 per student
1st Month Tuition Must be paid at the time of registration. Tuition will not be discounted if your dancer joins after the classes begin.
All parents/guardians are required to pay for classes in advance of the 1st of the month. There will be no exceptions. You may pay by credit, check or cash and make your checks payable to “Patty Flowerday School of Dance & Fitness”.
(There will be a 5% convenience fee for all credit card transactions.)
ALL TUITION IS DUE NO LATER THAN THE 5th DAY OF THE MONTH - On The 6th day the tuition increases $10 per class.

COVID-19 Notice and Acknowledgement Of Risk Form
The Covid-19 Virus is a serious and highly contagious disease. The world health organization has classified it as a pandemic. Our business wants to insure that you are aware of the additional risks of contracting Covid-19 and know Patty Flowerday School of Dance & Fitness is not liable for any diseases including Covid-19 while you or your dancer participates in dance class or any activities planned. If your dancer is ill please keep them home, they can make up their class at your convenience.
I confirm that I have read the notice above and understand and accept that there is a risk of contracting Covid-19 or any other virus while at Patty Flowerday School of Dance & Fitness. Initial: _______
By signing below, the registrant hereby agrees not to hold Patty Flowerday School of Dance & Fitness, its directors, and/or staff responsible for any damages, liabilities due to theft, accident of injury during or resulting from registrant or any relation thereof participating in any capacity of or related to any function or activity of the said Dance Studio. Registrant, parent or legal guardian hereby assumes ALL financial responsibility for the above named student enrolled at PFSD&F. Registrant understands the regular session (Sept-June) tuition is a commitment; therefore, PFSD&F is not liable for classes missed due to illness, holidays, pandemics or weather related closings. This tuition is a yearly fee divided into ten (10) installments. Payments are due by the 1st of the month. There is a $25.00 returned check fee. In addition if the account is delinquent the student will be asked to withdraw from the program for lack of payment. Your child’s picture may be used for advertising purposes for PFSD&F unless you specify otherwise.
