I have received or will obtain a copy of West Coast Dance Conservatory policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that West Coast Dance Conservatory does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc.

Tuition is based on one full school term (September-June), divided into 10 monthly payments. The first payment is due, along with registration fee, at the time of enrollment. Future monthly payments are due on the 1st. Although the number of classes may vary per month, the monthly fee remains constant and is based on the annual tuition amount. Tuition is divided into 10 equal payments for simplicity. Note that because tuition is based on a yearly cost, there is no pro-rating for shorter months. Statements will be sent via e-mail if your account is not paid by the 10th, in which case a late fee will be attached.

A 30 day notice is required for withdrawal and to stop automatic billing. All withdrawal requests must be submitted by email. We will bill your account 30 days from when we receive the notice. If your final day lands in the middle of a month, the final bill will be prorated. Once tuition is paid it is non-refundable.

ANNUAL PERFORMANCE: All students are automatically enrolled in the annual recital in June and the fees accepted with the performance. A dancers must opt out if they do not want to participate in the performance. The Spring Performance Fees will be sent out via email in the Fall of each year and charge to account on file with West Coast Dance Conservatory. Students must be enrolled full-time between January and June to participate in the Spring Performance. This is valuable class time utilized to learn the recital dance. If a dancer is not enrolled for any time during January to June, they will not be allowed to perform in the Spring Performance. All balances due in DanceStudio-Pro need to be payed in full by the time of the Spring Performance. Dancers will not be allowed to perform if they have an overdue balance. Costume and other recital-related fees are non-refundable.

CLASS LOCATION POLICY: West Coast Dance Conservatory is in beginning stages of renovating a NEW dance space 5 minutes away from the Downtown Millbrae Location in Burlingame. The new studio space is 5,000 square feet with 4 large dance studios! Once the the NEW additional studio space is complete in the Fall 2024 we will move the majority of classes to the new location. The location/studios for the classes may be subject to change once the new space is complete.

The COVID-19 pandemic and potential “shelter-in-place” orders implemented by state and local health authorities may impact our ability to offer in-person dance classes, camps, intensives, private lessons, competitions, conventions or the Spring Performance. These dance classes and other activities may transition to an online platform (i.e. Zoom) at any time. Tuition payments, fees and our withdrawal policies will remain the same for both in-person and online activities. No refunds, credits or pro-rated tuition will be offered if an alternative activity is available via an online platform (i.e. Zoom).

Dispute of a charge through your financial institution will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.

Photos taken in class, during performance or at any dance event, by a representative of West Coast Dance Conservatory may be used for any reasonable use for advertising and display purposes.

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event.

In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

I HEREBY WAIVE, RELEASE, AND DISCHARGE West Coast Dance and Fitness, LLC (DBA West Coast Dance Conservatory) and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics (i.e. COVID-19), accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives, private instruction, choreography or any activity I may participate.

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: West Coast Dance and Fitness, LLC and/or their directors, officers, managers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in this activity or event. This includes any claim based on negligence and/or strict liability.

The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old)

As parent and natural guardian of the enrolled student/dancer I hereby represent that I am, in fact, acting in such capacity, has consented to my child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. As a parent or guardian, I further agree to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.

Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
