I, ___________________________________, hereby grant Utah Dance Institute the right to take, edit, publish and distribute the photos and or videos taken of my student

This authorization extends to all media formats, now known or later discovered.
This authorization shall continue indefinitely unless I otherwise revoke this authorization in writing.
I waive the rights to inspect or approve any finished product in which my student appears.
I understand that these materials shall become the shared property of Utah Dance Institute the teacher/choreographer who is involved in the particular project, and the participating student.
