SBSDS MA Dance Private Summer Dance Camp

In addition to the SBSDS Hold Harmless Agreement/Liability and Photo and Media Release Waiver, this release form must be read and signed for every dancer participating in the SBSDS/ MA Dance/ Marching Auxiliaries Private Summer Camp.
I, the parent/guardian, hereby grant permission for MA to seek medical attention / treatment in case of illness or injury. I approve any attending physician to medically treat this child as deemed appropriate. I realize that any medical costs incurred due to illness/injury is my (parent or guardian) responsibility and not that of MA/Marching Auxiliaries, Inc. SBSDS and MA DANCE are not held responsible for any injuries, accidents, fees, etc.
Photo/Video Release Form
I grant to SBSDS and Marching Auxiliaries, Inc., the right to take photographs /video footage of me and my property in connection with this event. I authorize SBSDS and Marching Auxiliaries, Inc., its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that SBSDS and Marching Auxiliaries, Inc., may use such photographs and videos of my child with full permission from me, including for example such purposes as publicity, illustration, advertising, and Web content.
I grant SBSDS and Marching Auxiliaries, Inc. the right to photograph/video my child during this event and post on the company websites and social media.
I also grant permission for my child to attend SBSDS MA Dance Private Summer Dance Camp at the assigned facility and location.
Your electronic signature constitutes full acceptance of all conditions expressed in this release form. I have read and understand the above.
