Waiver of Liability
In consideration for the opportunity for your child, to participate in dance classes offered by Centennial State Ballet, the undersigned Parent/Guardian agrees to the following:
Assumption of Risk: I acknowledge that dance activities involve inherent risks, including but not limited to physical injury. I understand and accept these risks on behalf of my child.
Health and Fitness: I certify that my child is physically fit to participate in dance classes and has no medical conditions that would prevent their safe involvement in such activities. I agree to inform the Studio promptly of any changes to my child's health.
Emergency Medical Treatment: In the event of injury or illness during dance classes, I authorize the Studio's staff to seek emergency medical treatment for my child. I also agree to bear any expenses incurred for such treatment.
Release of Liability: I, on behalf of myself and my child, hereby release, discharge, and hold harmless Centennial State Ballet, its instructors, employees, and volunteers from any and all claims, liabilities, demands, actions, or causes of action that may arise out of or in connection with my child's participation in dance classes.
Attendance and Behavior: I understand that regular attendance and positive behavior are essential for a productive learning environment. I agree to support and enforce the Studio's policies regarding attendance, conduct, and dress code.
I have read and fully understand the terms of this Parental Waiver for Centennial State Ballet Dance Classes. I voluntarily agree to the terms and conditions stated herein.
