Heels Class Waiver Form
I accept the responsibility to ensure that my student is medically and physically fit to participate in this class. I also take the responsibility of checking the shoes worn before class to make sure they are adequate to use as equipment.
VOLENTI NON FIT INJURIA. {Latin: No wrong is done to one who consents.] I understand and accepts that this style of dance involves unusual physical demands on the human body my student may not be used to participating in and could be hazardous and accordingly I agree that I am aware of the risks and accept them and will personally bear all consequences which may be sustained as a result of participation in this class including all losses, costs, expenses, and medical expenses which may be sustained as a result of any injury.
I hereby expressly release Dancenter by Lori, Lori Ann Elder, all instructors, volunteers, dancers, or any other entities involved from any actions, claims, liabilities, losses, costs, or expenses which may result from such injuries. I further understand that Dancenter by Lori is not undertaking to provide the cost of any care or treatment that may be required in the event of injury and that, as between myself, my student, and Dancenter by Lori, any insurance against the result of any such injury or disciplinary action is solely my responsibility.
