I hereby authorize Starz Performing Arts Academy Inc. to charge my account the amount of $________ on the first day of each month starting on _____________ and ending on ______________.
Costume payments will also be charged to this card unless another form of payment is given before the due date. ___________ (initial)(costumes are non refundable)
*No credit or refunds on missed classes due to weather,holiday closures or any health closures. Classes can be made up in studio or virtually.
I will give the school office one month’s written notice from the first of the month to discontinue these charges.

*The dance school season is September- June or Summer session.It is assumed when a student registers, they are registering for the full dance season. However, your commitment is a billing term. Withdrawal from classes can only be done at the end of a billing term. Notice of withdrawal must be done in writing 30 days prior to the next billing term.X__________{initial)}
*I HAVE RECEIVED OR WILL OBTAIN A COPY OF STARZ PERFORMING ARTS ACADEMY POLICIES AND WILL TAKE THE RESPONSIBILITY TO CAREFULLY READ AND FOLLOW THE RULES AND POLICIES THEREIN. I UNDERSTAND THAT STARZ PERFORMING ARTS ACADEMY DOES NOT GIVE CREDIT AND/OR REFUNDS AFTER THE MONTH HAS BEEN PAID FOR, FOR CLASS MISSED DUE TO HOLIDAY, VACATION, ILLNESS, WEATHER, ETC. I FURTHER UNDERSTAND THAT THERE ARE SPECIFIC RISKS OF PHYSICAL OR PROPERTY DAMAGES, LOSSES, OR INJURY THAT MAY RESULT FROM MY OR MY CHILD’S PARTICIPATION WITH STARZ PERFORMING ARTS ACADEMY. I VOLUNTARILY ASSUME THE RISKS ASSOCIATED WITH SUCH PARTICIPATION AND HEREBY RELEASE THE STARZ PERFORMING ARTS ACADEMY FROM ANY AND ALL CLAIMS, COSTS, LIABILITIES, EXPENSES OR JUDGEMENTS ARISING OUT OF MY OR MY CHILD’S PARTICIPATION IN THE PROGRAM.
