Authorization & Release of Liability

I hereby authorize my child to participate in dance, tumbling, or pom classes.  I fully understand that these classes involve a wide and varied range of motion, physical activity, and movement, and I will not hold Dance Plus or any of its representatives, whether paid or volunteer, responsible in the unlikely event of injury to my child.

           

I have read the rules of the studio, and I am fully aware of the tuition rates, class rules, late fees, and other fees associated with Dance Plus.  I understand that I may forfeit my enrollment if rules are not followed, or fees are not paid.  Further, I understand that withdrawal from the studio does not release me from monies owed and that all fees will be paid before withdrawal.
